摘 要:目的:研究鼠胃cajal间质细胞(ICC)及缝隙连接蛋白在失血性休克复苏中的改变及意义。方法:随机数字表法将SD大鼠分为假手术组(SO)、休克复苏组(LR);分别于休克1 h,复苏3、6、12及24 h时相点在透视电镜下观察ICC的变化;实时荧光定量聚合酶链反应(PCR)分析缝隙连接蛋白(connexin,Cx)Cx43的改变。结果:透视电镜下观察,休克1 h ICC水肿,核皱缩,线粒体肿胀,胞浆内细胞器数量明显减少,基膜完整性破坏,细胞突起明显减少或消失;复苏3、6 h后细胞无明显变化;复苏12 h细胞肿胀减轻,胞浆内细胞器数量较前增多;复苏24 h细胞恢复接近正常。PCR检测缝隙连接蛋白Cx43mRNA水平从复苏3 h开始即呈逐渐升高趋势(P<0.05);复苏24h后达最大值与SO组无显著差异(P=0.147)。结论:失血性休克使ICC与缝隙连接蛋白受损;复苏早期ICC恢复较慢,而缝隙连接蛋白Cx43恢复较快,两者复苏过程并不同步,但在复苏24 h后接近正常。失血性休克复苏早期须重视ICC及缝隙连接蛋白功能的保护。Aim:To study the change of interstitial cajal cells(ICC)and connexin during resuscitation in hemorrhagic shock. Methods: SD rats were randomly divided into SO group and LR group.The ultrastructure of ICC was examined at different time points with transmission electron microscopy and the change of connexin 43 was detectd with PCR during development. Results: Electron microscopic examination revealed that serious destructions were observed in ICC,mitochondria swelled,nucleus concentrated and endoplasmic reticulum expanded and vacuolated,basal membranes destructed,cell processes decreased or disappeared.There was no significant change in ICC at 3 h and 6 h after resuscitation.The ultrastructure of ICC was gradually restored,the number of organels increased at 12 h and recovered to nearly normal level at 24 h after resuscitation.PCR analysis showed that connexin43mRNA levels obviously increased from 3 h,and reached at the peak at 24 h(P〈0.05),there was no significant difference between LR group and SO group(P=0.147). Conclusion:ICC and connexin43 were damaged in Hemorrhagic shock.ICC was delayed while connexin43 recovered rapidly in early stage after resuscitation.They did not synchronize during resuscitation,but they were both close to normal at 24 h after resuscitation.
