摘 要:目的探讨振动反应成像(vibration response imaging,VRI)中肺部定量数值(quantitative lungdata,QLD)在支气管哮喘诊断中的应用价值。方法选择支气管哮喘患者90例(哮喘组)和健康志愿者90例(对照组),均行肺功能及VRI检查,分析QLD值及其异常度、改善量,探讨其与肺功能的相关性。结果哮喘组支气管舒张试验前后各区域的QLD值异常度与对照组比较差异均无统计学意义(P>0.05);但哮喘组试验前后总QLD值异常度比较差异有统计学意义(P<0.01),哮喘组试验前总QLD值异常度与对照组比较差异有统计学意义(P<0.01)。哮喘组支气管舒张试验前后QLD改善量为7.6±2.2,95%可信区间为6.4~8.3。哮喘组支气管舒张试验前后QLD的改善量与FEV1的改善量呈线性相关(r=0.614,P<0.01),且与FEV1改善率也显著相关(r=0.691,P<0.01)。结论 VRI中的QLD值在支气管哮喘的诊断中具有一定的临床应用价值。Objective To explore the value of quantitative lung data(QLD) of vibration response imaging(VRI) in diagnosis of bronchial asthma.Methods The QLD and VRI examinations of 90 patients with bronchial asthma(group A) and 90 healthy volunteers(group B) were performed.The QLD and abnormity and improvement of QLD were analyzed.Results There was no statistical significance in abnormity of QLD in areas before and after bronchial relaxation test between group A and group B(P0.05).There was statistical significance in the total abnormity of QLD before and after bronchial relaxation test in group A(P0.01),and the total abnormity of QLD before bronchial relaxation test between group A and group B(P0.01).Improved amount of QLD in group A before and after bronchial relaxation test was 7.6±2.2,and 95% confidence region was 6.4~8.3.There was a linear relationship in group A before and after bronchial relaxation test between the improved amount of QLD and FEV1(r=0.614,P0.01),and also there was a significant relationship with improvement rate of FEV1(r=0.691,P0.01).Conclusion The QLD of VRI is of some value performs a promising method in diagnosis of patients with bronchial asthma.
