GnRH-a联合卡孕栓在子宫黏膜下肌瘤宫腔镜电切术中的应用

摘　　要：研究术前联合应用GnRH-a(达菲林)、卡孕栓对子宫黏膜下肌瘤宫腔镜电切术的疗效及并发症的影响。方法:2007年2月～2012年6月在我院住院的子宫黏膜下肌瘤患者共78例,其中观察组32例,术前应用GnRH-a(达菲林)3个月,术前30分钟阴道后穹窿放置卡孕栓1mg;对照组46例,术前30分钟阴道后穹窿放置卡孕栓1mg。结果:①最初确诊时两组肌瘤直径、瘤蒂直径、宫腔大小等比较,差异无显著意义(P>0.05),切除后两组肌瘤直径、瘤蒂直径比较,观察组直径明显缩小,两组比较差异有显著意义(P<0.01)。②观察组确诊时平均Hb74.5±7.3g/L,术前平均Hb113.2±6.8g/L,Hb平均上升35.7±7.1g/L。③观察组手术时间23±10min,出血量32±12ml,术中无一例并发症,无一例需中转开腹或行二期手术,术后月经均改善明显。对照组手术时间42±14min,出血量57±29ml,有3例出现并发症,2例中转开腹,仅两例中转开腹患者术后1、2月的经期延长。两组间比较,均P<0.05,有统计学差异。The study of preoperative combined application of GnRH- a （ Diphereline ）, methyl Carprost Suppository on submucosal myoma of uterus hysteroscopic surgery curative effect and complications of influence. Methods： from February, 2007 to June,2012 in our hospital 78 patients diagnosed of submucous myoma were divided into two groups, the observation group ： including 32 cases, applicated GnRH - a （ Diphereline ） 3 months before surgery, placed carprost 1 mg on the vaginal fornix 30 minutes before surgery; the control group： including 46 cases, placed carprost 1 mg on the vaginal fornix 30 minutes before surgery. Results： （~）initially in two groups there wes no difference of the diameter of myoma pediele,the diameter of my- oma,the size of the uterine cavity （ P 〉 0. 05 ） ,after resection the diameter of myoma pedicle, the diameter of myoma,were significantly reduced in observation group （ P 〈 0. 01 ）. In the observation group the average of Hb was 74. 5 ± 7.3g/L at first, the average of Hb was I 13.2 ± 6. 8g,/L before surgery, it rose 35.7 ± 7.1g/L. The operation time of observation group was 23 ± 10min,32 ±12ml bleeding volume,without complications, no patients required conversion to laparotomy or second stage surgery, postoperative period were improved obviously. The operation time of control group was 42 - 14rain,bleeding vol- ume of 57 ± 29 ml, 3 cases with complications, 2 cases were converted to laparotomy, ordy two cases laparotomy patients 1 and 2 months after the menstrual period extended. There was a statistically significant difference between two groups, P 〈 0. 05.
