颈前路手术治疗伴颈椎不稳的老年颈性眩晕的效果

摘　　要：目的观察颈前路手术治疗老年颈性眩晕的临床效果。方法 2006-01-2011-06采用不同方法治疗38名老年颈性眩晕患者,手术组：共20例,年龄61-78岁,平均69.7±4.97岁,全部采用颈前路手术治疗;对照组：共18例,年龄61-76岁,平均69.1±4.35岁,全部采用保守治疗。结果 38例患者均获随访,随访时间12～36个月,采用颈性眩晕症状与功能评分法进行评分,手术组：治疗前为13.75±3.34,治疗后3月、6月及最后一次随访分别为24.80±1.74、24.70±1.42、25.70±1.26;对照组：治疗前为13.94±3.56,治疗后3月、6月、最后一次随访分别为19.17±3.22、19.50±3.78、19.11±3.58,对两组数据采用SPSS 13.0进行分析,两组治疗效果有统计学差异,手术组优于对照组。结论颈前路手术可以较好重建颈椎稳定性,在老年颈性眩晕治疗中较保守方法治疗效果好。Objective To explore the efficacy of using anterior cervical surgery for the treat- ment of elderly cervical vertigo. Methods Thirty-eight elderly patients with cervical vertigo were managed from January 2006 to June 2011,and were divided into two groups. Operation groups （OG）：including 20 patients,whose age were 61-78 （average：69.7±4.97）,were treated by anterior route cervical discectomy and internal fixation. Control groups （CG）：including 18 patients, whose age were 61-76 （average：69.1±4.35）,were treated by conservation treatment. Results All patients were followed up for 12-36 months. According to Symptom and Function Score for cervical vertigo, he OG＇s scores pretherapy were 13.75±3.34,3 months after therpy were 24.80±1.74,6 months after therapy were 24.70±1.42,the last rechecking were 25.70±1.26 and the CG＇s scores pretherapy were 13.94±3.56,3 months after therpy were 19.17±3.22,6 months after therapy were 19.50±3.78,the last rechecking were 19.11±3.58. The differences between both groups were statistically significant （P〈0.01）. Conclusion Anterior cervical surgery can renew cervical stabilization. It is better than eanservative treatment elderly cervical vertigo patient.
