退变性腰椎侧弯并椎管狭窄症的两种手术方式疗效随访与对比研究

摘　　要：目的比较两种手术方式治疗退变性腰椎侧弯合并椎管狭窄症的临床指标,探讨单纯扩大潜式开窗减压手术治疗的临床疗效。方法搜集2006-01-2011-02间本院收治的病例,随机分两组:单纯扩大潜式开窗减压手术(A组)、后路减压植骨融合内固定术(B组)。同条件下,分析A组治疗此类疾病的优缺点。结果两组术前及终末随访的JOA、ODI、VAS评分和术前腰椎Cobb,s角比较无显著性差异(P>0.05)。两组手术时间、术中出血量、住院时间、费用和术后终末随访的腰椎Cobb,s角比较有显著统计学差异(P<0.05)。结论在严格掌握手术适应征条件下,A组方法有效又具有实用价值的手术方案。Object Through comparison of the two kind operative approach for the treatment of degenerative lumbar scoliosis merged spinal stenosis patients＇ clinical indicators. Exploring the potential to expend window decompression to clinical curative effect. Methods Collected during 2006-01-2011-02 patients,randomly divided into two groups：potential to expend window decom- pression（group A）,posterior decompression and lumbar interbody fusion（group B）. Under same conditions,A group the advantages and disadvantages of the treatment of this disease. Results Compared with two groups preoperatively and final follow-up of JOA,ODI,VAS score and Cobb＇s angle before operation were no significant difference （P〉0.05）. Compared with two groups of operation time,intraoperative blood loss,hospital stays,costs and postoperative follow-up of Cobb＇s angle were significant different （P〈0.05）. Conclusion Therefore, under the strict control of the indication of operation conditions,potential to expend window decompression （group A） treatment it is an effective and practical operation scheme.
