高场强MRI对梨状肌综合征的诊断价值

摘　　要：目的探讨高场强磁共振成像(MRI)对梨状肌综合征的诊断价值。方法分别使用高场强MRI、CT、超声和肌电图四种方法对55例已被临床证实的梨状肌综合征病例进行检查,并对四种检查方法所得出的结果施行统计学处理并进行分析比较。结果对梨状肌检查的结果显示MRI、肌电图、超声、CT的检出率(阳性率)分别为90.9%(50/55)、61.8%(34/55)、58.2%(32/55)、38.2%(21/55),漏诊率(阴性率)分别为9.1%(5/55)、38.2%(21/55)、41.8%(23/55)、61.8%(34/55);四种检查方法进行比较,MRI对病症的检出率(阳性率)明显高于其它三种检查方法,差异有统计学意义(P<0.01)。结论高场强MRI检查对梨状肌综合征具有无可比拟的诊断优势,能做到发现及时,定位准确,定性明晰,是该病最佳的检查手段。Objective To discuss the diagnostic value of high field strength magnetic resonance imaging （MRI） in detection of pirifonnis syndrome. Methods High field strength MRI,CT, Uhrasound and electromyography were used to check the 55 clinically proven cases of piriformis syndrome,detection results from above four methods were calculated,compared and statistical analysis. Results True detection rates（positive rates）of piriformis from MRI,electromyog raphy,uhrasound and CT were 90.9% （50/55）,61.8% （34/55）,58.2% （32/55）and 38.2% （21/55）;the misdiagnosis rates （false negative rates） were 9.1%（5/55）,38.2%（21/55）,41.8%（23/55） and 61.8%（34/55）;Among the four detecting methods MRI had significantly higher positive rate than other three methods. The differences were statistically significant （P〈0.01）. Conclusion High field strength MRI has unparalleled diagnostic advantage over other detective methods in diagnosis of piriformis due to its timely detection,accurately position and clearly qualification.
