老年健康男性慢性碘营养状况与亚临床甲状腺病的相关性研究

摘　　要：目的研究健康老年男性中的慢性碘营养状况与亚临床甲状腺疾病的关系。方法选择2010年4月在本科进行体检的年龄>75岁的老年男性进行膳食调查,通过公式计算每日摄碘量,分析入组人群的每日摄碘量与血清甲状腺激素水平、甲状腺超声改变以及亚临床甲状腺疾病的相关性,将入组人群分为碘适中亚组和碘足量亚组进行比较。结果健康碘营养正常的59例老年男性每日摄碘量与TSH、FT3、甲状腺容积以及甲状腺多发结节数量显著相关(P<0.05)。入组人群中碘适中亚组和碘足量亚组比较,TSH水平显著降低,FT3水平显著增高,甲状腺平均体积显著减少,甲状腺结节数量显著减少,亚临床甲状腺疾病发生率显著降低。结论高龄老年男性的碘营养需求可能发生随年龄的降低,碘营养正常范围内的碘足量摄入即增加亚临床甲状腺疾病的风险。Objective To study the correlation between chronic iodine nutrition status and subclinical thyropathy in healthy elderly male group. Methods Healthy males aged over 75 who performed physical examination in our department in April 2010 were selected. The detailed daily diets were ＇investigated and daily iodine intake were estimated referring to standard formula. In the selected group, correlation between daily iodine intake and serum thyroxine level, Bultrasonic thyroid capacity and the incidence of subclinical thyroid diseases were analyzed. Moderate and sufficient subgroups divided by iodine nutrition were compared with each other. Results Fifty-nine eligible elderly males were enrolled. Significant correlations were shown between daily iodine intake and the level of serum TSH, serum FT3, the capacity of ~hyroid and the quantities of the thyroid nodules （P 〈 0.05 ）. Compared the iodine moderate subgroup with the sufficient subgroup, the level of TSH significantly decreased while the level of FT3 significantly increased, average capacity of thyroid significantly enlarged with decreased numbers of the thyroid nodules. An increased incidence of subclinical thyropathy was found in iodine sufficient subgroup compared to moderate subgroup. Conclusion Daily iodine requirement might be decreased by age, which resulted in the higher risk of suffering from the subclinical thyroid diseases in the elderly, especially with sufficient iodine intake
