摘 要:目的:建立严重多发伤动物模型,检验现场救护装备实验效果。方法:健康比格犬31只,体质量12.5~15.0 kg,随机分为严重多发伤、出血性休克模型组(A组,n=18),和严重多发伤、胸腹联合伤、上呼吸道窒息模型组(B组,n=13)。A组再分为休克组(A1组,n=6),单一液体复苏组(A2组,n=6),液体+自体血回输复苏组(A3组,n=6);B组再分为全梗阻窒息≤6 min组(B1组,n=4),全梗阻窒息7~8 min组(B2组,n=4)和全梗阻窒息≥9 min组(B3组,n=5),对比观察各组的实验效果。结果:A1组动物全部死亡,A2组动物半数死亡,A3组动物全部存活;B1组动物全部存活,B2组动物死亡1例,B3组动物死亡2例,B组死亡动物均为使用常规救生器械复苏者。结论:抢救严重多发伤、失血性休克,液体复苏+自体血回输优于单一液体复苏组,综合救生装备明显优于常规器械。Objective:To establish an animal model of severe multiple trauma and test the experimental effect of on-site life supporting devices.Methods: Thirty one healthy Beagles of 12.5-15.0 kg were randomly divided into 2 groups: group of severe multiple trauma and haemodynamic shock model(Group A,n=18),group of severe multiple trauma,thoraco-abdominal injury and of upper respiratory tract obstruction asphysia model(Group B,n=13).Group A was further divided into 3 sub-groups: shock group(Group A1,n=6),single fluid resuscitation group(Group A2,n=6) and fluid plus autotransfusion group(Group A3,n=6).Group B was further divided into 3 sub-groups: group of full obstruction and suffocation ≤6 mins(Group B1,n=4),group of full obstruction and suffocation 7-8 mins(Group B2,n=4) and group of full obstruction and suffocation≥9 mins(Group B3,n=5).Experimental effect of each group was compared.Results: All the animals of Group A1 were dead,half of Group A2 were dead while animals of Group A3 all survived;animals of Group B1 all survived,1 animal was dead in Group B2 and 2 were dead in Group B3.The dead animals in Group B were all the ones having used the conventional apparatus.Conclusion: Effect of fluid resuscitation plus autotransfusion group are superior to that of single fluid resuscitation group in rescuring severe multiple trauma and haemodynamic shock,effect of comprehensive life supporting device group is obviously superior to that of conventional apparatus group.
