三维斑点追踪技术评价正常成人左室三维扭转功能的初步研究

摘　　要：目的探讨三维斑点追踪成像新技术评价正常成人左室三维扭转功能的可行性及其正常参考值和影响因素。方法采用三维斑点追踪成像技术测定102例正常成人的左室心肌旋转运动,获得以下指标:(1)左室基底段、中间段和心尖段平均旋转角度(rotation);(2)左室整体扭转度(twist);(3)左室整体扭力(torsion)。结果左室心尖段旋转度显著高于中间段和基底段,而中间段扭转度显著高于基底段。正常成人左室整体扭转度为10.58°±3.52°,左室整体扭力为(2.04°±0.76°)/cm。性别、年龄、体质量指数、血压、血糖、血脂等常见因素均与左室扭转功能无明显相关性。结论三维斑点追踪技术评价左室扭转功能耗时较短,具有高度可行性,可望成为评价左室扭转功能的有效新手段。Objective To explore the feasibility, normal reference value and influence factors of thre-dimensional speck- le tracking echocardiography in evaluating left ventricular 3-dimensional twist function in the normal subjects. Methods The left ventricular twist function in 102 normal subjects was evaluated using three-dimensional speckle tracking echocardiography to acquire the following indices, （1）average rotation angle of left ventricular basal, middle and apical segment, respectively. （2） left ventricular global twist. （3）left ventricnlar global torsion. Results Rotation degree of apical segment was significantly higher than that of middle and basal segment, and the rotation degree of middle segment was significantly higher than that of basal segment. The left ventricular twist and torsion was （10.58±3.52） degree and （2.04±0. 76） degree/cm in normal sub- jects, respectively. The common risk factors such as gender, age, body mass index, blood pressure, blood glucose and blood cholesterol were not related to left ventrieular twist function. Conclusions Evaluating left ventricular twist function using three-dimensional speckle tracking echocardiography was highly feasible and may be a novel and effective tool in evaluating left ventricular twist function.
