急诊集束化治疗对急性心力衰竭患者心肺功能的影响研究

摘　　要：目的探讨急诊集束化治疗对急性心力衰竭患者心肺功能的影响。方法选取2011年1月—2012年6月我院急诊科就诊的采用集束化治疗的严重急性左心衰竭患者60例为集束化组,另选取2009年1月—2010年12月我院急诊科就诊的采用常规处理的严重急性左心衰竭患者58例为对照组。入急诊室后均行常规处理,集束化组在此基础上给予集束化治疗,观察两组治疗前后呼吸(RR)、动脉血氧饱和度(SaO2)、动脉血氧分压(PaO2)、收缩压(SBP)的变化及气促缓解时间。结果集束化组与对照组治疗前RR、SaO2、PaO2和SBP比较,差异均无统计学意义(P>0.05);治疗后集束化组较对照组RR减慢,SaO2、PaO2升高,SBP降低,差异均有统计学意义(P<0.05)。集束化组气促缓解时间(4.4±2.6)h短于对照组的(8.4±3.3)h,差异有统计学意义(t=7.383,P<0.001)。结论急诊集束化治疗能快速提高患者的SaO2及PaO2,显著缩短气促缓解时间,对患者短期内症状的改善和心肺功能的稳定疗效显著,值得临床推广。Objective To observe the influence of bundle treatment on cardiopulmonary function of patients with acute heart failure. Methods 60 patients with severe acute left ventricular failure underwent bundle treatment in emergency depart- ment of our hospital from January 2011 to June 2012 were selected as bundle treatment group, while 58 patients with severe acute left ventricular failure underwent conventional treatment from January 2009 to December 2010 were selected as control group. Both groups were given routine treatment after admission, and the bundle treatment group was also given bundle treatment. The RR, SaO2, PaO2, SBP and remission time of breath shortness of the two groups were observed. Results RR, SaO2, PaO2 and SBP between bundle treatment and control group before treatment showed no statistically significant differences （P 〉 0.05 ）. After treatment, the RR in bundle treatment group was slowed, the SaO2 and PaO2 were increased and the SBP was decreased （P 〈 0. 05）. The remission time of breath shortness in bundle treatment group （4. 4±2. 6） h was shorter than that of the control group （8.4 ＋ 3.3） h （ t = 7. 383, P 〈 0. 001）. Conclusion Bundle treatment can quickly improve patients＇ SaO2 and PaO2, significantly shorten the remission time of breath shortness and achieve satisfactory effect on patients＇ short term symptoms and car- diopulmonary function stability.
