摘 要:目的探讨慢性肾衰竭(CRF)患者血清Chemerin水平及其与肾功能之间的关系。方法收集2010年6月—2012年3月在我院门诊就诊或住院的慢性肾脏病3、4期患者158例作为CRF组,选择同期行体格检查的健康体检者140例作为对照组,采用酶联免疫吸附法(ELISA法)检测两组受试者的血清Chemerin和超敏C反应蛋白(hs-CRP)水平;核素动态显像法测定肾小球滤过率(GFR);生化方法测定血尿素氮(BUN)、血肌酐(Scr),并观察上述指标与血清Chemerin水平之间的关系。结果 CRF组患者血清Chemerin水平为(101.3±31.7)μg/L,高于对照组的(68.4±25.4)μg/L,差异有统计学意义(t=2.656,P<0.01);hs-CRP水平为(3.80±0.88)g/L,高于对照组的(1.60±0.76)g/L,差异亦有统计学意义(t=2.241,P<0.05)。CRF患者血清Chemerin水平与GFR呈负相关(r=-0.58,P<0.01),与BUN、Scr、hs-CRP水平均呈正相关(r=0.43,P<0.05;r=0.51,P<0.05;r=0.41,P<0.01)。多元逐步回归分析发现GFR是影响血清Chemerin水平的独立相关因素。结论 CRF患者的血清Chemerin水平与肾功能有关,随肾功能的降低,其血清Chemerin水平升高。Objective To investigate serum levels of adipokine chemerin in patients with chronic renal failure (CRF) and its relationship to renal function. Methods One hundred and fifty - eight 3 - 4 stage CRF outpatients and patients admitted to hospital from June 2010 to March 2012 were enrolled as CRF group, and 140 healthy subjects receiving physical examinations at corresponding period as control group. Enzymes linked immunosorbent assay (ELISA) was used to determine serum chemerin and high sensitive C - reactive protein (hs - CRP), radionuelide dynamic imaging to measure gIomerular filtration rate ( GFR), and biochemical method to detect blood urea nitrogen ( BUN), serum creatinine (Set). The relationship of the above indexes to serum chemerin was observed. Results Serum Chemerin and hs - CRP levels were higher in CRF group than in control group [ (101.3±31.7) μg/Lvs. (68.4±25.4) μg/L, t=2.656, P〈0.01 and(3.80±0.88) g/Lvs. (1.60±0.76) g/L, t = 2. 241, P 〈 0.05 1. Serum Chemerin was negatively correlated with GFR ( r = - 0. 58, P 〈 0. 01 ) , and positively with BUN, Scrandhs-CRP (r=0.43, P〈0.05; r=0.51, P〈0.05; r=0.41, P〈0.01). The multiple stepwise regression analysis showed that GFR was independent variables of serum Chemerin. Conclusion Chemerin level is related to renal function in CRF patients.
