神经营养因子及扳机点排尿治疗女性糖尿病神经源性膀胱的临床疗效分析

摘　　要：目的观察评价神经营养因子及扳机点排尿综合疗法对女性糖尿病所致神经源性膀胱的临床疗效。方法选取2008年1月—2011年5月于我院治疗的35例女性糖尿病神经源性膀胱尿潴留患者,其中20例采用神经节苷酯肌肉注射及扳机点排尿综合治疗(综合治疗组);8例采用单纯扳机点排尿治疗(单纯治疗组);7例因治疗费用问题采用定时导尿治疗(对照组)。比较3组患者治疗前后残余尿量变化情况、菌尿发生情况。结果随访3～5个月,3组患者治疗前后残余尿量比较,差异均无统计学意义(P>0.05)。综合治疗组发生菌尿2例,对照组发生菌尿3例,单纯治疗组无发生,3组患者菌尿发生率比较,差异有统计学意义(P<0.05)。结论对于女性糖尿病神经源性膀胱患者给予神经营养因子及扳机点排尿综合治疗,效果理想,且并发症少,为一种较好的治疗方法。Objective To observe the clinical efficacy of neurotrophic factor and trigger point urinating in treating women with diabetic neurogenie bladder. Methods 35 female patients with diabetic neurogenic bladder and urinary retention ad- mitted to our hospital from January 2008 to May 2011 were selected. 20 cases were given intramuscular injection of ganglioside and trigger point therapy （comprehensive treatment group） ; 8 cases were only given, trigger point therapy （single treatment group） and 7 cases were given catheterization treatment due to financial reasons （control group） . Residual urine volume and incidence of bacteriuria between the three groups before and after treatment were compared. Results According to the three to five months fol- lowing up, the residual urine volume between the three groups showed no statistically significant differences （ P 〉 0. 05 ） . 2 cases in comprehensive treatment group had bacteriuria, 3 cases in control group had bacteriuria, while there was no bacteriuria in sin- gle treatment group, and the difference between the three groups was statistically significant （ P 〈 0. 05 ） . Conclusion Com- bined treatment of neurotrophie factor and trigger points urination can achieve satisfactory efficacy with less complications for women with diabetic neurogenic bladder.
