行为转变理论模式在社区老年糖尿病患者行为改变中的应用

摘　　要：目的探讨行为转变理论在帮助2型糖尿病患者建立健康行为中的作用。方法选择184例社区老年2型糖尿病患者作为研究对象,随机分为干预组和对照组。干预组接受行为转变理论为理论基础的护理干预措施,对照组接受常规的健康教育。干预前和实施护理干预6个月后,采用调查问卷表进行评定。结果干预后干预组糖尿病相关知识的答对率高于干预前,并高于对照组(P<0.05);对照组的答对率在干预前后差异无统计学意义(P>0.05);干预后干预组患者糖尿病相关知识知晓率高于对照组(P<0.05),并高于同组治疗前的知晓率(P<0.05);干预前后两组患者BMI无明显变化(P>0.05),两组间干预后患者BMI也无明显变化(P>0.05);对照组患者干预前后空腹血糖无明显变化(P>0.05),干预后干预组患者空腹血糖明显下降(P<0.05)。结论行为转变理论模式对帮助社区老年2型糖尿病患者有效控制血糖水平,建立良好的健康行为具有重要义。Objective To evaluate the effectiveness of nursing intervention based on trans-behavior theoretical model on the behavior change of patients with type 2 diabetes.Methods A total of 184 community elderly patients with type 2 diabetes were randomly divided into two groups —the intervention group and the control group.The patients in the intervention group received nursing intervention measures based on the trans-behavior theoretical model,while the patients in the control group received normal healthy education.Before the intervention and six months after the intervention,the Diabetes Questionnaire was used to evaluate the results.Results After the intervention,the answer reply of diabetes-related knowledge in intervention group was significantly higher than that before the intervention,and higher than that in control group（P0.05）;Before and after the intervention,there was no significant difference in control group（P0.05）;After the intervention,the related knowledge awareness rate was significantly higher in intervention group than that in control group（P0.05）;after the intervention,there was no significant difference in BMI between the two groups（P0.05）;Before and after the intervention there was no significant change in FBG in the control group（P0.05）;Patient＇s FBG in intervention group decreased significantly after the intervention（P0.05）.Conclusion The trans-behavior theoretical model has great significance in helping the elderly patients with type 2 diabetes in effectively controlling the blood glucose and establishing good healthy behavior.
