社区高血压网络管理平台应用效果分析

摘　　要：目的:使用社区高血压网络管理平台对社区高血压患者进行管理,并对其应用效果进行分析。方法:对康健社区卫生服务中心服务范围内的300名高血压患者通过高血压网络平台实施管理,并与传统社区高血压管理的患者进行控制率比较。结果:社区高血压网络管理平台患者血压控制率为97.33%,传统管理组患者血压控制率为97.04%,两组无统计学差异(P>0.01)。结论:社区高血压网络管理平台作为现有社区高血压慢病管理模式的补充和完善,可有效应用于社区高血压的慢病管理。Objective： The network management platform was used for the management of community hypertension patients and its application effect was analyzed. Method： Three hundred patients with hypertension in Kangjian community were managed with the network management platform and the control rate was compared with that of the traditional management method. Result： The control rate of patients blood pressure under the community network management platform was 97.33% while 97.04% under the traditional management, and there were no statistically differences（P 〉 0.01）. Conclusion： The networkmanagement platform for community hypertension management can be used as the supplement and perfection of the existing community hypertension management mode and can be effectively applied to the chronic disease management of hypertension in community.
