平衡针刺结合功能训练对脑卒中后肩-手综合征的疗效观察

摘　　要：目的探讨平衡针刺结合康复功能训练对脑卒中后肩一手综合征的影响。方法将120例脑卒中偏瘫合并肩一手综合征患者随机分为治疗组和对照A、B组，其中治疗组40例采用康复功能训练结合平衡针灸治疗，对照A组40例采用常规康复功能训练，对照B组40例采用平衡针刺疗法。治疗前及治疗后2、3、4周分别采用现在疼痛状况评定（Present Pain Intensity，PPI）、Fugl—Meyer运动功能评定量表（Vugl—meyer Motorfunction Assessment，FMA）、改良Barthel指数评定量表fModified Barthel Index，MBI）评价患者的功能。结果治疗2周后，三组患者的上肢及手的运动功能均有改善（P〈0．05），组问比较治疗组的效果优于对照A、B组（P〈0．05）治疗3周后，三组患者的上肢及手的运动功能均有改善（P〈0．05），组间比较治疗组的效果优于对照A组（P〈0．05）、治疗4周后，j组患者的上肢及手的运动功能均有改善（P〈0．05）。结论平衡针灸结合康复功能训练可较快改善脑卒中后肩一手综合征症状，改善患者的上肢功能。Objective To explore the effects of balance acupuncture combined with function training in treatment of shoulder-hand syndrome after brain stroke. Methods 120 cases were randomly divided into three groups： 40 cases in treatment group were treated with the rehabilitation function training combined with balance acupuncture, 40 cases in control group A were treated with conventional functional training 40 cases in control group B were treated with balance acupuncture. All patients were assessed before and after therapy in two weeks, three weeks and four weeks by present pain intensity （PPI）, Fugl- Meyer motor function assessment （FMA） of the upper limb and modified barthel index （MBI）. Results The scores of the three groups were improved after two weeks（P 〈 0.05）, but the score of the treatment group was significantly better than those of the control group A and the control group B （P〈 0.05）. There was no significant difference between the control group A and B （P〉 0,05）. After three weeks, the score there was no significant difference between the treatment group and the control group B（P〉 0.05） , but the control group A and B was significantly different （P 〈 0.05）. After four weeks, the score of each group showed no significant difference （P〉 0.05）. Conclusion Balance acupuncture combined with functional training can rapidly improve the upper-limb function and shoulder-hand syndrome after brain stroke.
