摘 要:目的观察沙丁氨醇吸入对机械通气新生儿肺透明膜病(HMD)患儿的影响。方法治疗组机械通气HMD患儿吸入沙丁胺醇,在给药前及给药后1h测定呼吸力学及血气指标并进行比较,统计并比较两组患儿机械通气时间及支气管肺发育不良(BPD)的发生率。结果治疗组用药后呼吸系统静态顺应性(Cstatic)升高(P〈0.01)氧合指数(OI)、吸气阻力(Rinsp)、呼吸功fWOB)及二氧化碳分乐(PaCO2)下降沪〈0.01)。治疗组与对照组机械通气时间分别为(86.53±22.88)h、(104.81±23.69)h;治疗组与对照组BPD发生率分别为6.25%、16.13%。结论机械通气HMD患儿吸入沙丁胺醇,可短期改善肺顺应性,降低气道阻力,增加通气/血流,减少呼吸功,缩短机械通气时间,但不能降低BPD的发病率。Objective To observe the effects of salbutamol aerosolinhaled in infants with ventilated hyaline membrane disease of newborn (HMD). Methods The HMD infants with mechanical ventilation treated by aerosol salbutamol were studied as treatment group. The indexes of respiratory mechanics and blood gas were measured and compared prior and 1 hour after aerosol delivery. The mechanical ventilation time and the bronchopulmonary dysplasia (BPD) incidence of inhalant group and control group were compared. Results The static compliance (Cstatic) of respiratory system after administration were significantly higher than those before administration ( P〈 0.01 ).The oxygenation index (OI), inspiratory resistance (Rinsp), work of breathing( WOB )and the partial pressure of carbon dioxide in artery( PaCO2 ) after administration were significantly decreased compared with those before inhalation(P 〈 0.01).The mechanical ventilation time of the treatment group was( 86.53 ± 22.88 )h, while that of the control group was ( 104.81 ± 23.69 )h, there was statistical significance (P〈0.01). The BPD incidence of the treatment group and control group were 6.25%(2/32) and 16.13%(5/31)respectively, there was no statistical significance (P〉0.05). Conclusions Salbutamol inhalation in ventilated neonatal respiratory distress syndrome infants can improve lung compliance and reduce airway resistance.The ventilation/perfusion ratio is increased, while the WOB is decreased, and the mechanical ventilation time is shortened in a short time, but the BPD incidence can not be reduced.
