脑脊液中腺苷酸脱氨酶活性检测在结核性脑膜炎临床诊断和预后判断中的价值

摘　　要：目的探讨脑脊液中腺苷酸脱氨酶(ADA)检测对结核性脑膜炎(TBM)临床诊断和预后判断的价值。方法检测127例TBM患者(TBM组)和102例非TBM患者(非TBM组)脑脊液中ADA的活性,分析ADA在TBM临床诊断中的价值及其与患者预后的关系。结果 TBM组患者脑脊液中ADA活性明显高于非TBM组(P<0.05),预后良好患者脑脊液中ADA活性明显低于预后不良患者(P<0.05),Hosmer-Lemeshow拟合优度检验显示ADA活性对TBM预后结局的预测具有较好的校准度。结论 ADA活性在TBM的临床诊断及预后判断中具有一定的应用价值,可作为临床诊断TBM及预后判断的辅助检测指标。Objective To explore the value of adenosine deaminase（ADA） activity in diagnosis and predicting prognosis of patients with tuberculous meningitis（TBM）.Methods ADA activity in cerebrospinal fluid of 127 patients with TBM（TBM group） and 102 patients without TBM（non-TBM group） was detected,then value of ADA activity in diagnosis and predicting prognosis of patients with TBM was analyzed.Results The activity of ADA in TBM group was significantly higher than that in non-TBM group（P0.05）,and the activity of ADA in patients with favourable prognosis was significantly lower than that in patients with unfavourable prognosis（P0.05）.The results Hosmer-Lemeshow test showed that calibration of ADA activity on the prognostic outcome prediction of patients with TBM was better.Conclusion The activity of ADA in clinical diagnosis and predicting prognosis in patients with TBM has certain value,ADA can be used as a index of clinical diagnosis of TBM and prognosis of patients with TBM.
