摘 要:目的探讨骨科无痛病房建设中的临床应用效果。方法给予患者按时镇痛,明确组织架构与职责,进行持续性的人员培训、教育与考核,制订及实施疼痛控制程序,落实疼痛教育,进行阶段总结与反馈。结果实施无痛病房管理模式后,医护在疼痛评估、疼痛一般知识、药物镇痛知识、疼痛知识的综合应用4个指标的得分均高于此模式建立前,差异有统计学意义(P<0.05)。实施无痛病房管理后,患者接受调查时、过去24 h、手术后所经历的最严重疼痛程度均在5分以下(0~10级评分);患者对疼痛治疗方式、疼痛照顾、止痛效果的满意度评分在4分以上(5级评分),患者满意度评分高于实施前,差异有统计学意义(P<0.05)。结论骨科无痛病房管理的实施提高了医护的疼痛管理水平,提高了患者的疼痛控制质量,使患者得到了优质的无痛服务。Objective To develop a pain management practice of establishing thopaedic painless of the patients was benefited from pain a painless unit in orthopedic urelief on timing method in orunit procedures. A pain management practice of establishing a painless unit were developed in orthopedic unit. The measure were carried out including painless unit management, the painless unit organization, painless unit human resource assignment and the responsibilities of painless unit different positions. These steps were established and implemented such as painless unit staff training, painless unit staff continuing education, and pain assessment management. Pain management program were followed and reviewed regularly. Results After the pain management practice of establishing a painless unit, the painless unit staff' knowledge were improved significantly about pain assessment of painless unit, pain general knowledge, knowledge about pain control medication, as well as comprehensive application of pain knowledge. The difference in above these knowledge had statistical significance, compared to these before the establishment of the painless unit. The patient loaded pain score at the moment of evaluation, in the 24 hours before the moment of evaluation, and the most severe postoperative pain were no more than 5 (score ranged from 0 to 10). Patient's satisfaction rate on the treatment method against pain, pain management and analgesia effect were more than 4 (full score was 5). There was significant difference in patient' s satisfaction about the painless unit staff quality prior to and after the implementation of new model of painless unit management practice. Conclusion The model of painless unit management practice improve the painless unit staff quality on conducting the standard procedures for pain management. Meanwhile, Patient' s satisfaction rate on the treatment is achieved.
