血清降钙素原、D-二聚体联合检测在门诊治疗社区获得性肺炎中的应用

摘　　要：目的探讨血清降钙素原(PCT)、D-二聚体联合检测在门诊治疗社区获得性肺炎(community-acquiredpneumonia,CAP)中的应用价值。方法收集2009年6月～2011年11月在门诊接受治疗的CAP患者32例,另随机抽取30例健康体检者为对照组,治疗前及第8天清晨采集静脉血检测PCT及D-二聚体,同时进行PSI评分并分组,其中PSI I级12例,PSIⅡ级12例,PSIⅢ级8例,并对各组PCT及D-二聚体的检测结果进行比较。结果CAP组血清PCT及D-二聚体水平明显高于对照组(P<0.01),CAP组治疗后PCT及D-二聚体水平明显低于治疗前(P<0.01);CAP患者PSI分组中,各组PCT、D-二聚体水平明显高于对照组,差异有统计学意义(P<0.01,P<0.05),PSIⅢ级组高于PSIⅠ级组及Ⅱ级组(P<0.01,P<0.05)。结论血清PCT与D-二聚体对CAP有诊断意义,与CAP的严重程度有一定的相关性。因此检测二者血清水平可以帮助临床医师早期诊断CAP,为正确判断病情,合理治疗提供参考依据。Objective To investigate the clinical significance of combined detection with procalci- tonin and D-dimer in serum in treating community-acquired pneumonia（CAP）at outpatient de- partment. Methods The serum of 32 patients with community-acquired pneumonia treated at the outpatient department was collected, and the serum of 30 healthy people was randomly collected as the control group. Before treatment and in the early morning of eighth day, venous blood was collected to test PCI and D-dimmer. At the same time, PSI score was calculated and classified. Results The levels of procalcitonin and D- dimer in serum of CAP group had sta- tistical difference compared with the healthy group （P〈0.01）. The levels of procalcitonin and D -dimer in serum of CAP group before therapy had statistical difference compared with that af- ter therapy, （P〈0.01）. Conclusion Serum PCT and D-dimer has significant value in diagnosis, and is related to the severity of CAP. So the detection of procalcitonin and D-dimer may help the clinician with early diagnosis of CAP in that it can provide reference for the evaluation of severity of illness and rational therapy.
