摘 要:目的探讨逆转录-聚合酶链反应(RT-PCR)直接从临床麻疹患者的尿液中检测麻疹病毒特异性核蛋白(Measles vi-rus nucleoprotein,MV-N)基因诊断价值。方法选取2010-2011年安徽省蚌埠市散发的15例临床确诊为麻疹的患者,采集尿液,提取RNA,用RT-PCR方法扩增麻疹病毒核蛋白的515 bp核苷酸片段。同时采集同一患者血清,进行MV-IgM抗体检测,并比较两种方法的阳性率。结果尿液RT-PCR法检测MV-N基因,15例患者中有14例扩增出阳性产物,阳性率为93.3%,高于血清MV-IgM抗体检测的阳性率60.0%(χ2=9.60,P=0.002)。结论麻疹病人尿液标本的RT-PCR检测可用于麻疹病毒的早期快速辅助诊断,特别是对于出疹初期的患者。同时对血清MV-IgM抗体阴性的病例进行此法复检,可弥补漏检。Objective To explore the clinical value of directly detecting measles virus-specific nucleoprotein gene(MV-N)from clinically diagnosed measles patients' urine using reverse transcription-polymerase chain reaction(RT-PCR).Methods Urine was collected from 15 patients clinically diagnosed with measles from 2010 to 2011 in Bengbu,Anhui province.RNA was extracted from urine.RT-PCR was used to amplify 515 bp nucleotide fragment of the measles virus nucleoprotein.Serum of the same patient was collected to detect MV-IgM antibody.The positive rate was compared between two methods.Results Positive products were successfully amplified in 14 of 15 patients by RT-PCR method.The positive rate was higher by RT-PCR method than by MV-IgM detection method(93.3% vs 60.0%,χ2=9.60,P=0.002).Conclusion RT-PCR for detecting the urine can be used as an early fast-assisted diagnosis method for measles virus,especially for patients with early rash.In the meantime,re-examination of negative MV-IgM in serum could reduce the missed diagnosis.
