慢性再生障碍性贫血患者中医证型与相关因素分析研究

摘　　要：[目的]通过对慢性再生障碍性贫血(chronic aplastic anemia,CAA)患者中医证型分布及其诸多相关因素的分析,探讨其主要影响因素,以期对CAA的临床辨证治疗有一定的现实指导意义,并为进一步研究提供理论依据。[方法]选取符合CAA诊断标准的CAA患者76例,收集患者的一般情况、临床症状及血常规、网织红细胞、骨髓增生度、分析其中医证型,评价贫血积分,并填表记录。同时,抽取患者外周血5mL,ELISA法检测促红细胞生成素(Erythropoietin,EPO)、干扰素(Interferon,INF),构建CAA临床信息数据库,应用logistic回归分析中医证型与相关因素的关系。[结果]患病人群以中青年为主,男性略多于女性,病程较长,证型分布以肾阴阳两虚型最多,而证型分布与年龄、性别和病程没有明显相关性;与肾阳虚相关度较大的为血红蛋白、白细胞、EPO,与血红蛋白呈负相关;与白细胞、EPO呈正相关;与肾阴虚相关度较大的是血小板,与其呈负相关;与肾阴阳两虚相关的是白细胞、EPO,与其呈负相关。[结论]白细胞、血红蛋白、血小板、EPO对CAA的证型具有重要的影响,值得在今后的临床和科研工作中引起重视并进一步深入研究。[Objective] With analysis on TCM types distribution and many relative factors of CAA,to explore its main influencing factors,have a definite practical meaning for its clinical differentiating treatment and offer theoretic base for further research.[Method] Select standard 76 patients,collect their general conditions,clinical symptoms,blood tests,reticulocyte and myelosis degree,analyse TCM types,evaluate anemia score,and fill in a form for record.Meanwhile,draw peripheral blood（PB） 5ml,take ELISA method to test EPO and INF,construct CAA clinical database and apply logistic regression analysis to the relation with concerned factors.[Result] The patients are mostly young and middle-aged persons,male more than female,with longer course,mostly with both deficiency of kidney Yin and Yang,and the types distribution is not markedly related with age,sex and course;the hemoglobin,white cells and EPO are much concerned with kidney Yang deficiency;the platelets are much related with kidney Yin deficiency in negative relation;white cells and EPO are related with kidney Yin and Yang deficiency in negative relation.[Conclusion] White cells,hemoglobin,platelets and EPO have important effect on CAA types,worthy further study in later clinical and research work.
