亚临床甲减对老年2型糖尿病患者视网膜病变的影响

摘　　要：目的探讨亚临床甲状腺功能减退(甲减)对老年2型糖尿病视网膜病变的影响及他汀类药物对视网膜病变的影响。方法纳入400例≥65岁的老年2型糖尿病患者,其中亚临床甲减45例,甲状腺功能(甲功)正常组共355例。筛查患者糖尿病视网膜病变。观察2组间糖尿病视网膜病变发生率的差异,并初步探讨不同甲功状态对糖尿病视网膜发病的影响,同时观察他汀对糖尿病视网膜病变的影响。结果亚临床甲减组糖尿病视网膜病变的发生率相较于甲功正常组显著下降,但在排除他汀的影响后,上述2组间的差异无统计学意义;logistic回归分析提示他汀对于老年糖尿病合并亚临床甲减人群糖尿病视网膜病变有独立保护作用。结论亚临床甲减与老年2型糖尿病视网膜病变的发生无明确相关,他汀类药物对老年2型糖尿病合并亚临床甲减患者视网膜病变的发生起保护作用。Objective To discuss the association between the subclinical hypothyroidism and diabetic retinopathy, and study the effect of statins on diabetic retinopathy. Methods A total of 400 type 2 diabetic subjects were enrolled： 45 subjects with subclinical hypothyroidism and 355 subjects without. The prevalence of diabetic retinopathy （DR） in the two groups, and the effect of statins on DR were observed and analyzed. Results The prevalence of DR in subclinical hypothyroidism was significantly higher than that in euthyroid group. However, after removing the effect of statins by logistic regression analysis, there was no significant difference between the two groups, and the statins had a protective role in DR. Conclusions There is no significant correlation between the subclinical hypothyroidism and DR, and statins can play a protective role in DR.
