双胎试管婴儿临床分析

摘　　要：目的探讨双胎试管婴儿母亲妊娠合并症及新生儿临床结局。方法收集2010年3月～2012年6月住院的双胎46例,体外受精-胚胎移植术后23例,自然受孕23例(均为双绒毛膜双羊膜囊双胎),对产妇年龄、妊娠周数、分娩方式、妊娠期合并症、新生儿体重、新生儿围生期疾病进行比较分析。结果试管组孕妇平均年龄及剖宫产率高于自然受孕组,试管组孕妇妊高征及双胎选择性胎儿宫内生长受限的发生率均高于自然受孕组。试管组孕妇妊娠期糖尿病、早产、胎膜早破、贫血的发生率与自然受孕组无明显差异。试管组双胎儿的新生儿高胆红素血症发生率高于自然受孕组,平均胎龄、平均体重与自然受孕组无明显差异。感染、缺血缺氧性脑病、坏死性小肠结肠炎、肺透明膜病、窒息的发生率与自然受孕组无明显差异。结论应加强IVF-ET术后孕妇及胎儿的监护,防止新生儿各种疾病发生。Objective Explore the twin test-tube baby mother pregnancy complications and neonatal clinical outcomes. Methods Collected between March 2010 and my twin 46 cases in the hospital, after in vitro fertilization-embryo transplantation in 23 cases, naturally fertilized in 23 cases （both dual-chorionic twin amniotic sac twins）, maternal age, the number of weeks of pregnancy, childbirth, pregnancy complications, birth weight, neonatal perinatal diseases for comparative analysis. Results IVF.ET twin pregnancies and cesarean section rate higher than the average maternal age naturally fertilized group. IVF ET postoperative twin pregnant women with pregnancy-induced hypertension and twin selective intrauterine growth restriction were higher than the natural conception group. Test-tube group of gestational diabetes, preterm labor, premature rupture of membranes, anemia incidence in the control group no significant difference. IVF ET double-fetal neonatal hyperbilirubinemia was higher than the natural conception group, mean gestational age, average weight and the control group, no significant difference. Infection, hypoxic-ischemic encephalopathy, necrotizing enterocolitis, hyaline membrane disease, asphyxia was no significant difference with the control group. Conclusion Postoperative IVF- ET in pregnant women and fetal monitoring should be strenghende, prevention of various diseases, so as to preventing neonatal diaeases.
