摘 要:[目的]探索研究阿仑膦酸钠和洛发他汀预防大鼠早期激素性股骨头坏死的疗效。[方法]将120只SD大鼠随机分成四组,A组:30只,阿仑膦酸钠干预;B组:30只,洛伐他汀干预;C组:30只,阿仑膦酸钠+洛伐他汀干预;D组:30只,生理盐水干预,干预2周后连续注射大剂量甲基强的松龙4周。注射结束后12周处死大鼠行HE染色组织病理学检测和Micro-CT检测研究骨小梁微观结构变化情况。[结果]通过计算骨小梁空骨陷窝率得出A、B、C和D组大鼠股骨头坏死发生率分别为33.3%、40.0%、23.3%和56.7%。同时四组的Mirco-CT结果显示A、B、C组的骨小梁平均数目及平均厚度显著高于D组,骨小梁平均间距显著低于D组(P<0.01);且C组的骨小梁平均数目及平均厚度显著高于A、B组,骨小梁平均间距显著低于A、B组(P<0.05),A、B两组结果之间差别无统计学意义(P>0.05)。[结论]阿仑膦酸钠和洛伐他汀可以增加大鼠股骨头骨小梁的密度,从而对大鼠早期激素性股骨头坏死的防治具有一定作用,且两者联合应用的效果更明显。[ Objective] To study the effects of alendronate and lovastatin in preventing early glucocorticoids -induced osteonecrosis of femoral head (ONFH) in rats by micro- CT. [ Methods] Totally 120 SD rats were divided into four groups randomly, 30 rats were in group A garaged with alendronate solution, 30 rats were in group B gavaged with lovastatin solution, 30 rats were in group C gavaged with alendronate and lovastatin solution, and 30 rats were in group D ( control group) gavaged with physiological saline tor two weeks. And then, all rats were injected intraperitoneally high - dose of methylprednisolone tot 4 weeks continuously. At the time of 12th weeks after intervention, all rats were sacrificed tor hematoxylin and eosin staining (HE) to cal- culate the success rates of glucoeorticoid - induced osteonecrosis of femoral head model in rates and scanned the trabeculaes of femoral head by Micro - CT. [ Results] The success rate of glucocorticoid - induced osteonecrosis of femoral head animal model in group A was only 33.3 percent, only 40 percent in group B, and only 23.3 percent in group C, but about 56.7 percent in group D. At the time of 12th week after intervention, the micro -CT results of group A, B, C had statistica| significance (Pa 〈 0. 01 ) , comparing to the group D. Comparing to the group C, the micro - CT results of group A or B had statistical significance ( P 〈 0. 05) . Between group A and B, the micro - CT results also had no significance (P 〉 0. 05) . [ Conclusion] Ac- cording to the micro - CT results, alendronate or lovastatin plays a certain role in the preventing of osteonecrosis of femoral head at an early stage caused by glucocorticoids in rats through increasing the trabecular plate density. Furthermore, combined application of alendronate and lovastatin has better effect than simply application of alendronate or lovastatiu in the preventing of glucocorticoids - induced osteonecrosis of femoral head.
