双心室起搏治疗心力衰竭的初步经验

摘　　要：目的　介绍双心室起搏治疗心力衰竭的初步经验。　方法　 6例充血性心力衰竭患者 ,男性 4例 ,女性 2例 ,平均年龄 5 8岁 ;心功能 NYHA分级 : ～ 级 ;均伴有心室内阻滞。患者均植入了三腔双心室起搏器 ,左心室导线通过冠状静脉窦插入心脏静脉侧分支或侧后分支。　结果　所有患者植入起搏器后临床症状改善 ,超声心动图检查显示左心室收缩功能和同步性改善 ,舒张期充盈改善 ,二尖瓣返流减少 ,此外 ,双心室起搏后 ,所有病例 QRS时限缩短。　结论　双心室起搏对于药物难治性心力衰竭 ,伴有 QRS波增宽的患者 ,可使心功能得到改善。Objective To introduce the preliminary experience of biventricular pacing to treat congestive heart failure. Methods Six patients,male in 4 and female in 2,mean age of 58 years old,with congestive heart failure and left bundle branch block or intraventricular delay were recruited.All patients received biventricular pacing.The left ventricular pacing lead was inserted into the lateral branch or potero lateral branch of cardiac vein via coronary sinus to pace the left ventricle. Results After biventricular pacing,the clinical symptoms were improved in all patients.The improvement of heart function and reduction of mitral regurgitation were found in echocardiogram. Conclusion Biventricular pacing could improve the heart function in patients with congestive heart failure and left bundle branch block.
