摘 要:目的探讨前列地尔预处理对移植肝的保护作用及可能的机制。方法 30例终末期肝病患者随机分为两组:A组供体摘取后以UW液灌注,同时以前列地尔10μg经门静脉灌注,4℃保存;B组供体摘取后常规以4℃UW液灌注保存。两组均在供肝恢复血流后3h抽取外周静脉血检测肝功能,并取部分肝组织检测髓过氧化酶(MPO)、超氧化物歧化酶(SOD)活性及肿瘤坏死因子α(TNF-α)水平,计算细胞凋亡指数,观察肝组织病理改变。结果 A组肝组织中MPO活性和TNF-α水平低于B组(P<0.05),而SOD活性高于B组(P<0.05)。A组肝细胞凋亡指数低于B组。结论前列地尔预处理可通过减少供肝组织TNF-α释放、降低炎症反应而减轻供肝的缺血-再灌注损伤和肝细胞凋亡。ObjectiVe To study the protective effect and underlying mechanism of lipo- prostaglandin E1 preconditioning on grafted liver. Methods Thirty patients with end-stage liver disease were equally randomized into two groups. The donor liver of group A was perfused with 10 μg lipo-prostaglandin E1 and 4℃ UW solution via portal vein, which of group B with 4℃ UW solution only. The cold ischemia time was 10 hours. Liver functions(ALT and AST), myeloperoxidase(MPO) and superoxide dismutase(SOD) activities,and tumor necrosis factor -a(TNF-a) were assessed at 3, 24,48 and 72 h after liver transplantation. Pathological changes and TUNEL apoptosis staining were observed. Results The MPO activity and TNF-a expression were lower, but SOD activity was higher, in group A than those in group B(P〈0. 05). The apoptosis index of hepatic cells was lower in group A than that in group B(P〈0. 05). Conclusion Lipo-prostaglandin E1 preconditioning may protect the grafted liver from I-R injury possibly through reducing the release of TNF-a and decreasing inflammation.
