肥厚型心肌病的心电图与MRI表现分析

摘　　要：目的探讨肥厚型心肌病(hpertrophic crdiomyopathy,HCM)的心电图和MRI表现。方法搜集17例经临床确诊的HCM患者资料,对其心电图、MRI表现进行总结分析。结果 17例患者心电图均异常,主要表现为ST-T异常、左束支传导阻滞、左心室高电压、异常Q波、T波倒置。MRI以左心室壁非对称性肥厚、室间隔肥厚居多,其次是心尖肥厚;心腔缩小,心肌信号正常;心脏功能异常(7例)。ST段压低深度与心尖部室壁厚度呈正相关,Q波深度与室间隔厚度呈正相关,T波深度与心尖部和室间隔的室壁厚度差呈正相关。结论 HCM的心电图、MRI均有一定的特征性,两者结合、相互补充,可为临床诊断该病提供重要的依据。Objective To Analysis analyze the electrocardiogram and MRI features performances of hypertrophic cardio- myopathy（HCM） ＇s electrocardiogram and MRI. Methods The electrocardiogram and MRI features of 17Collect 17 cases patientsof with HCM confirmed by clinical, were analysis analyzedthe performances of electrocardiogram and MRI. Results All the cases are had abnormal electrocardiogram results, included main show is ST-T anomaly,left bundle branch block, left ventricular high voltage, abnormal Q wave,T wave inversion. The major MRI features weremain show is left ventricular wall asymmetry hypertrophy and interventricular hypertrophy, follow bysecondly were apex hypertrophy;, heart cavity nar- rowedstenosis,. The MRI signal of myocardialum was signal normal;, part ofSome cases had heart dysfunction. The depth of ST ST segment down depressiondepth and had positive correlation with apex ventricular wall thickness were positive corre- late. The depth of Q wave depth andhad positive correlation with interventricular thickness were positive correlate. The depth of T wave had positive correlation with T wave depth and apex and interventricular thickness were positive correlate. Conclusion MRI and electrocardiogram have characteristic performances features of in HCM have certain characteristic, MRI combine combind with electrocardiogram two methods can provide important information for clinical correct diagnosis.
