摘  要:目的探讨加入左卡尼汀(LCN)的St.Thomas No.2液对大鼠离体心脏长时间冷保存的效果及其机制。方法利用Langendorff灌注装置建立离体大鼠心脏灌注和工作模型,SD大鼠24只随机分成3组,每组8只,对照组采用St.Thomas No.2液作为心脏停搏液并作为保存液保存离体心脏7h,LCN1组、LCN2组采用St.Thomas No.2液分别添加6g/L、12g/L左卡尼汀作为心脏停搏液并作为保存液保存离体心脏7h,观察各组离体心脏保存前及保存后再灌注后心功能情况,比较心功能恢复率、心脏复跳时间、心肌含水量及心肌组织匀浆丙二醛的变化。结果各组心脏保存7h后均能成功复跳;3组之间心脏复跳时间比较差异无统计学意义(P﹥0.05);心功能指标以LCN2组最好,LCN1组次之,对照组最差(P﹤0.05);3组心肌含水量比较,对照组含水量最高,LCN1组次之,LCN2组最低,差异有统计学意义(P﹤0.05)。丙二醛以对照组最高(P﹤0.05),LCN1组、LCN1组比较差别无统计学意义(P﹥0.05)。结论左卡尼汀可以强化St.Thomas No.2液长时间冷保存大鼠离体心脏的保存效果,此效果与左卡尼汀减轻心肌缺血再灌注损伤有关。OBJECTIVE To observe the protective effects of St. Thomas No.2 solution supplemented with L-carnitine (LCN) on prolonged hypothermic preservative rat heart. METHODS Isolated rat heart Langendorff model and working model were es- tablished. Twenty-four Sprague-Dawley rats were randomized divided into three groups (8 rats in each group) : Control group (the hearts were arrested with 4℃St. Thomas No.2 solution and preserved in the same solution for 7 h), LCN1 and LCN2 group (cardioplegic solutions and organ preservation solutions were 4℃St. Thomas No.2 solution supplemented with LCN 6g/L or 12g/ L for 7 h). The isolated hearts were perfused in the Langendorff perfusion apparatus for the left ventrieular funetional evaluation before and after preservation. The recovery of myocardial funetion, malondialdehyde (MDA) and wet weights in myoeardium were analyzed. RESULTS After the hearts were preserved for 7 h, there were no differences in the time for heart resuseitation in the three groups, but LCN2 group showed the best recovery of myocardial function, the lowest release of MDA, and the lest wet weights in myoeardium in the three groups (P 〈 0.05 ). CONCLUSION St. Thomas No.2 solution supplemented with L-ear- nitine provides better myocardial protective effects over purely cardioplegie solutions for the prolonged hypothermie preservative isolated rat heart.
