胃异位胰腺MSCT表现与病理对照分析

摘　　要：目的探讨胃异位胰腺的多排螺旋CT的影像学特点,并与病理对照分析。方法回顾性分析经多排螺旋CT(multislice spiral CT,MSCT)平扫及双期增强检查和手术病理证实的胃异位胰腺18例患者的资料,分析其CT表现特征。结果胃异位胰腺18例中,发生于胃底2例,胃体3例,其中大弯侧2例,小弯侧1例;胃窦部13例,其中大弯侧9例,小弯侧4例。病变均单发,卵圆形或圆形,边缘不清,与胃壁广基底相连。13例向腔内生长,3例位于胃周脂肪内,另外2例因病灶较小生长方式难以判断。增强扫描3例显著均匀强化,病理以胰腺腺泡为主要成分;13例明显强化,其强化接近于正常胰腺,病理显示胰腺腺泡、导管混合存在;另外2例强化不明显,病理以胰腺导管和平滑肌为主要成分。3例显示中央脐凹征。结论胃异位胰腺CT表现具有一定的影像学特点,不同的强化方式与异位胰腺的病理组成相关。Objective Multi-slice spiral CT imaging features of gastric ectopic pancreas, and pathological according to an- alyze. Methods A retrospective analysis of the enhanced inspection and pathologically confirmed 18 cases of gastric ec- topic pancreas in patients with multi-slice spiral CT scan and dual-phase, analyzed the CT features. Results The gastric ectopic pancreas in 18 cases,occurred in the fundic two cases,three cases of gastric body,including two cases in the greater curvature,lesser curvature one cases ; 13 cases of gastric antrum, greater curvature of the nine cases, the lesser cur- vature side four cases. Lesion was solitary ,small oval or round mass ,the edge was unclear,and connected to the stomach wall wide substrate. 13 cases of cavity growth, and three cases in the perigastric fat,the other two cases due to the smaller growth of the lesion is difficult to judge. Enhanced scan three cases of significant homogeneous enhancement, pathology of the pancreatic acinar as the main ingredient; 13 cases significantly strengthen its enhanced close to the normal pancreas, pathological pancreatic acini, catheter mixed exist; enhancement was the other two cases,the pathology of the pancreatic duct as the main ingredient. Three cases of central umbilical depressions sign. Conclusion CT findings of gastric ectopic pancreas has a certain image features, enhanced with ectopic pancreas pathology composition related.
