右美托咪定对腰硬联合麻醉患者术中镇静效应的影响

摘　　要：目的探讨右美托咪定肌肉注射对腰硬联合麻醉患者术中镇静效果的影响。方法选择2011年8月～2012年10月在腰硬联合麻醉下行手术治疗的患者120例为研究对象,随机分为对照组和研究组(S1组,S2组,S3组,S4组)。对照组未肌肉注射右美托咪定。研究组在麻醉成功后肌肉注射右美托咪定1.5μg/kg、2.0μg/kg、2.5μg/kg、3.0μg/kg,观察生命体征及不良反应情况。结果 S2、S3、S4组镇静达到高峰时间比较差异有统计学意义(P<0.01),对应的BIS值三组间比较,差异有统计学意义(P<0.01)。各组达最大镇静效果时警觉/镇静评分分布情况差异有显著统计学意义(P<0.01)。S1与S2组出现低血压的比例高于对照组,S3和S4组出现窦性心动过缓的比例高于对照组,差异均有统计学意义(P<0.01)。S4组出现过度镇静的例数显著高于其他组,差异有统计学意义(P<0.01)。结论右美托咪定应用于临床麻醉具有抑制应激、稳定循环、辅助麻醉等作用。Objective To discuss effect of dexmedetomidine on sedation of patients undergone combined spinal and epidu- ral anesthesia. Methods Selected 120 cases undergone combined spinal and epidural anesthesia from Aug 2011 to Oct 2012 were divided into study group （S1, S2, S3, S4） and control group. Control group wasn＇t given dexmedetomidine. Study group was injected dexmedetomidine with 1.5 μg/kg, 2.0 μg/kg, 2.5 μg/kg, 3.0 μg/kg after anesthesia. Vital signs and un toward effect were observed. Results Calm peak time and BIS in S2, S3, S4 group had significant difference （P 〈 0.01）. OAM S score in S1, S2, S3, S4 group was significant difference （P 〈 0.01）, Hypotension cases in S1,S2 group were more than con- trol group, and sinus bradycardia cases in S3,S4 group were more than control group （P 〈 0.01）. Excessive calm cases in S4 group were more than other groups （P 〈 0.01）. Conclusion Dexmedetomidine for clinical anesthesia shows inhibiting stress, stable circulation, supplemental anesthesia.
