摘  要:目的对万古霉素在治疗大鼠肺炎模型不同给药方案进行评价。方法 MRSA标准菌株ATCC43300为试验菌株,构建大鼠肺炎模型,依给药剂量分为四个治疗组:Ⅰ组(40 mg·kg-1,qd),Ⅱ组(20 mg·kg-1,qd),Ⅲ组(10 mg·kg-1,qd),Ⅳ组(20mg·kg-1,bid),并于感染后16 h采用万古霉素治疗,治疗后10,20,30,60,90,120 min采集血液标本,微生物法测定大鼠体内的血药浓度,分析不同给药方案的疗效。结果各治疗组的生存率随给药剂量增加逐渐增高,但差异无统计学意义(P>0.05);各治疗组肺组织匀浆菌落计数皆低于对照组(P<0.05);Ⅰ组和Ⅱ组、Ⅳ组比较差异无显著性(P>0.05);Ⅰ组、Ⅱ组和Ⅲ组比较有显著性差异(P<0.05)。结论万古霉素的负荷量达到一定的峰浓度时,其疗效不在随着药物浓度的增加而增加,每天两次给药与一次给药在治疗效果上差异无显著性。Objective To evaluate different dosing regimen of vancomycin in treating MRSA pneumonia of rat model.Methods The rats which were infected with standard strains of ATCC43300 for pneumonia were divided into four groups,according to the administration of different dosages:groupⅠ(40 mg·kg-1,qd),group Ⅱ(20 mg·kg-1,qd),group Ⅲ(10 mg·kg-1,qd),group Ⅳ(20 mg·kg-1,bid).Plasma samples were collected at 10,20,30,60,90,120 min after drug administration,concentrations in serum were determined by using microbiological assays,the effects of different drug dosage were evaluated.Results The statistics showed that on the whole of rats in each group the survival rate gradually increased with the increased dose,but the difference was not statistically significant(P0.05).The colony counts in lung homogenates of all treatment groups were lower than those of the model group(P0.05);the bacterial counts of the lungs in groupⅠ and groupⅡ,group Ⅳ had no significant difference(P0.05);however they,had significant difference among groupⅠ,groupⅡand group Ⅲ(P0.05).Conclusions The efficacy doesn't rise with the increased drug concentration after reaching a certain peak concentration of vancomycin.There was no significant difference in efficacy in the twice daily dosing with the single dose.
