良肢位摆放对脑梗死偏瘫患者早期康复的影响

摘　　要：目的探讨良肢位摆放对急性脑梗死患者偏瘫侧肢体并发症、运动功能及日常生活自理能力(ADL)的影响。方法将符合研究条件的急性脑梗死患者按入院顺序分为试验组和对照组,试验组170例,对照组180例。入院后两组均接受相同的神经内科常规治疗及护理。试验组同时全程采用床上良肢位摆放,对照组采用功能体位。4周后观察并比较两组偏瘫侧肢体并发症的发生、运动功能和日常生活自理能力情况。结果试验组患者偏瘫侧肢体并发症发生率显著低于对照组(P<0.01)。运动功能及生活自理能力改善程度与对照组比较差异有显著性(P<0.05)。结论对急性脑梗死患者偏瘫侧肢体早期全程进行良肢位摆放,能减少偏瘫侧肢体并发症的发生,减少肢体的痉挛和挛缩,改善肢体运动功能,促进瘫痪肢体的康复,从而提高患者的生活自理能力,提高生活质量。Objective To explore the influences of normal limb position in hemiplegic limb complications,motor functions,or activities of daily living（ADL） on patients with acute cerebral infarction.Methods A total of 350 cases were randomly divided into patient group and control group（170 cases in patient group,180 cases in control group）.Both groups were treated with conventional therapy and neurological rehabilitation.For the study group,normal limb position was required.After 4 weeks,the scores of complications,motor function and the ADL were compared between two groups.Results The complication rate of study group was significantly lower than that of control group.There was significant difference in scores of motor function and the ADL between two groups.Conclusions Normal limb position is effective for prevention of joint contracture and reduction of complications in patients with hemiplegia.It can also promote the rehabilitation of paralyzed limb and improve the quality of life.
