摘 要:目的:了解创伤性脑损伤(traumatic brain injury,TBI)后脑组织匀浆中一氧化氮(nitric oxide,NO)含量、一氧化氮合酶(nitric oxide synthase,NOS)活性以及与脑水肿之间的关系。方法:36只SD大鼠,随机分为两组:正常对照组(n=6)、手术组(n=30)、手术组按动物处死不同时间(6、24、72、120、168 h进行分组,每组6只。上述各组取脑组织匀浆检测NO及NOS,及测定脑组织含水量。结果:(1)TBI后脑组织内NO含量、NOS活力即有升高,在伤后6 h即有升高,72 h明显升高,并一直处于较高水平,与对照组比较差异有统计学意义(P<0.05);(2)脑组织含水量在外伤后升高,与对照组比较差异有统计学意义(P<0.05)。与脑组织NO含量、NOS活力变化趋势一致。结论:(1)大鼠TBI后损伤灶外确实发生了NO、NOS的升高,伤后1周内持续存在;(2)NO对脑水肿的发生有重要作用。提示在临床处理脑损伤中有必要早期补充外源性NOS清除剂,与脱水剂联合使用,可以减轻脑水肿继发性损伤。Objective: To determine and evaluate the changes of NO and NOS in brain tissue after traumatic brain injury and relationship between them and brain edema,at the same time to detect the changes of NO and NOS in serum and relationship between these changes and positive NOS neurons.Methods: Thirty-six SD rats were used and divided into two groups:normal control and operation group.The cerebral contusion model was produced by dropping weight.The traumatic brain injury model was built successfully and contents of NO and NOS in cerebral contusion tissue were measured,so were brain water contents.Results:(1)Data of normal control and operation groups were significant difference(P0.05).(2)NO values of the traumatic right hemisphere were significant higher than that of normal control group,so were brain water contents and NO in serum(P0.05).NOS activety of the traumatic hemisphere was higher than that of the operation group(P0.05).Conclusions: The results of this study suggests that(1)NO played important role in tissue damage due to ischemia and bleeding.(2)NO cause brain edema and vice versa.
