乳腺癌改良根治术术前心理状况分析及术后心理干预

摘　　要：目的通过对乳腺癌患者行改良根治术前的心理分析,探讨术后心理干预对乳腺癌康复的影响。方法采用症状自量表(SCL-90)连续对100例诊断乳腺癌明确、欲行改良根治术的患者进行术前的心理测评,并与中国健康常模组进行比较。术后康复期患者随机分成对照组50例和干预组50例,两组均随访6个月,干预组定期进行心理干预,对照组则不作任何处理,采用焦虑自评量表(SAS)、抑郁自评量表(SDS)及癌症康复评价简表(CABES-SF)对两组患者心理状态及生活质量进行评定。结果乳腺癌患者术前普遍存在焦虑、抑郁等心理问题,SCL-90的6个因子均分、总分及阳性项目总数均高于国内常模,差异有统计学意义。术后心理干预后患者各量表评分均明显低于对照组(P<0.05)。结论乳腺癌术前心理评估非常必要,术后心理干预有助于改善患者的情绪障碍、心理健康及生活质量。Objective To explore the effects of pre-operative psychological analysis and post-operative psychological intervention in breast cancer patients with modified radical mastectomy.Methods One hundred patients who were diagnosed with breast cancer and would be given modified radical mastectomy were given SCL-90 questionnaire for preoperative psychological evaluation,and then compared with China health norm group.Postoperative patients in rehabilitation period were randomly divided into control group （50 cases） and intervention group （50 cases）,two groups were all followed up for 6 months,the intervention group was given regular psychological intervention,and control group with no intervention.Then,SAS,SDS and CABES-SF questionnaires were used to evaluate psychological status and quality of life of two groups.Results Breast cancer patients generally have anxiety,depression and other psychological problems.SCL-90 six factor split,the total score and total positive project were higher than those of the domestic norm group,the difference was statistically significant.Each scale scores of patients with postoperative mental intervention were significantly lower than that in control group （P 0.05）.Conclusion Pre-operative psychological analysis in breast cancer patients is essential,and post-operative psychological intervention can effectively help them to overcome these emotional disorders,and improve their mental health and quality of life.
