摘  要:目的探讨重组人粒细胞集落刺激因子(recombinant human granulocyte colony stimulating factor,rhG-CSF)动员后联合骨髓干细胞移植治疗糖尿病下肢血管病变的有效性和安全性。方法选择30例糖尿病下肢血管病变患者,采用rhG-CSF动员后联合骨髓干细胞对患肢进行治疗,以治疗前作为自身对照;治疗后1~6个月观察患者肢体肤色、皮温、肢体麻木疼痛、间歇性跛行、踝肱指数(ankle brachial index,ABI)及经皮氧分压(transcutaneouspressure of oxygen,TcPO_2)的变化。结果治疗后患者肢体皮肤颜色、皮温、肢体麻木疼痛症状、间歇性跛行及ABI、TcPO_2均较治疗前明显改善,且无相关并发症和不良反应发生。结论 rhG-CSF动员后联合骨髓干细胞移植可有效治疗糖尿病下肢血管病变,此治疗方法安全、有效,能提高患者的生活质量。Objective To evaluate the effect and safety of bone marrow stem cell transplantation combined with hypodermic injection of recombinant human granulocyte - colony stimulating factor ( rhG - CSF) in treating diabetic lower limb ischemia. Methods Thirty patients with diabetic lower limb ischemia diseases were enrolled in this study. The patients received rhG - CSF and bone marrow stem cell transplantation by intramuscular injections at multiple sites of affected limb. During 1 - 6 months of follow - up, skin color, skin temperature, sensation, claudication, ankle brachial index ( ABI), and transcutaneous pressure of oxygen ( TcPO2 ) were observed and compared with pre - treatment. Results The symptoms were greatly relieved 1 month after treatment. The ABI and TcPO2 were increased from 0. 45 ± 0.11 to 0.69 ± 0.13 and 18 ± 2 to 25 ± 2 ( P 〈 0.01 ) 6 months after stem cell transplantation treatment, respectively. Conclusion rhG - CSF combined with bone marrow stem cell transplantation is an effective and feasible method for the treatment of diabetic lower limb ischemia disease.
