某一甲医院农村COPD住院患者医疗费用变动情况及其相关因素分析

摘　　要：目的分析新型农村合作医疗实行前后及新医改药品零差价后农村慢性阻塞性肺疾病(COPD)患者住院费用构成和变动情况。方法回顾性分析山东省某"一甲"医院2003～2010年1908例COPD住院患者的总费用和构成情况,分析医疗费用变动情况及其影响因素。结果自2003年起,例均治疗费用和检查费逐年上升,2010年达高峰。例均药费和例均住院费自2003年起逐年上升,2009年达峰值。2010年新医改后实行药品零差价后药费和住院费下降明显,药费和住院费增长趋势有所控制,护理费变化不明显。结论实行新型农村合作医疗及新医改药品零差价可降低COPD患者住院费用的支出。在控制住院费用增长的过程中治疗费、检查费和药费是需要控制的因素,应完善相关的农村医疗制度。Objective To analyze composition and changes of the hospitalization cost of the rural COPD patients before and after new rural cooperative medical scheme and drug′s zero-profit policy.Methods The total cost and composition of 1908 COPD patients from 2003 to 2010 in a certain class A hospital were reviewed to analyze the changes and the influencing factors.Results From 2003,the treatment and test cost per case increased year after year and reached the peak value in 2010.After 2003 the hospitalization cost and drug expenditure increased year after year and reached the peak value in 2009.But after drug′s zero-profit policy in new medical reform.The hospitalization cost and drug expenditure obviously declined in 2010,however the changes of nursing cost was not distinct.After drug′s zero-profit was implemented,the increased trend of the drug expenditure and the cost of hospitalization was controlled.Conclusion The practice of new rural cooperative medical scheme and drug′s zero-profit policy in new medical reform could reduce the hospitalization cost of the COPD patients.The cost of treatment,test and the drug expenditure were important factors in controlling the cost.And the related rural medical system should be improved.
