摘 要:目的:测量蒙古族人正常髌骨的解剖学参数,为国人建立髌骨数据库和设计符合国人膝关节假体提供参考。方法:蒙古族青年志愿者60人,男18人,女42人,120个髌骨,行CT薄层扫描,所得DICOM数据应用Mimics10.01软件处理,重建膝关节3D数字化模型,测量髌骨高度(H),髌骨宽度(W),髌骨最厚处厚度(T),髌骨最薄处厚度(T1)。数据用SPSS软件进行统计学处理及相关分析。结果:H=40.59±3.26(51.28-34.65),W=43.63±3.70(58.09-37.35),T=21.42±1.84(27.56-16.92),T1=15.96±1.42(21.18-13.53);正常蒙古族人髌骨宽髌型为多(100/120);侧别间、性别间比较无显著性差异(P值均>0.05),身高和各径线有线性正相关性。以15mm为界,T1小于15mm者占28.3%(34/120),髌骨切除关节面后骨床的厚度很难达到15mm;54个髌骨T小于21mm,即使使用8mm髌骨假体也可能总厚度和术前髌骨厚度不一致;T小于20mm者占22%(27/120),使用最薄的假体(8mm),也要超过原髌骨厚度。结论:置换市售的髌骨假体易出现置换后总厚度超过原厚度的情况应予以注意,在术前应用CT检查测得髌骨数据,行个性化截骨置换合适型号假体更为妥当。Objective: To found patellar database and design prosthesis of knee joints for the Chinese people, through measuring the anatomic parameter of the normal patella in Mongolian people. Methods :The height ( H), width ( W), maximal (T) and minimam thickness ( T1 ) of the patella in 60 adult (including 18 male and 42 famale) from Mongolian people were 3D-measured by CT and Mimics 10. O1 software. All data were analyzed by SPSS software. Results:All patellar parameter were nosignificant difference between two sides and the sexes (P 〉 0.05 ). Correlation between patellar parameter and height was statistical significant. According to measured,the height (H) is 40.59±3.26 mm, ranging from 51.28mm to 34.65mm, the width (W) is 43.63±3.70mm, ranging from 58.09mm to 37.35mm,the maximal thickness (T) is 21.42±1.84ram,ranging from 27.56mm to 16.92mm,the minimam thickness ( T1 ) is 15.96± 1.42 mm, ranging from 21.18 mm to 13.53 ram. The wide patella were more than others in Mongolian people. 28.3% of the parameters of patellar minimam thickness (T1) were less than 15mm (34/120), so patellar thickness is difficult to achieve to 15ram after resurfacing. T were less than 21mm in 54 patella, so using 8mm patellar prosthesis was inconsistent with the preoperative patellar thickness. 22% of the parameters of patellar maximal thickness (T) were less than 21mm (27/120), so using 8mm patellar prosthesis maybe beyong the preoperative patellar thickness. Conclusion:The problem should be noticed that patellar prosthesis beyong the preoperative patellar thickness. We should measure patellar parameter by CT preoperation and choose individual osteotomy and patellar prosthesis
