摘  要:目的探讨微创切口与普通切口植入起搏器电极在制作犬慢性快速心房起搏房颤模型中的差异,为找寻更好的犬模型制作方法提供依据。方法选取健康成年家犬12只(1~2岁),雌雄不限,体质量(16±0.5)kg。随机分为普通切口组(6只)与微创切口组(6只),分别按普通切口术式和微创切口术式植入起搏器电极,建立慢性快速心房起搏房颤模型。术中观察犬出血量、手术时间,术后以400次/min连续起搏8w诱导心房颤动。术后观察犬的一般情况,定期监测心电图、心脏彩超,记录犬的肢体导联心电图变化,观察心房颤动的发生情况。结果 12只家犬均手术成功。普通切口组1只家犬因囊袋血肿于术后第1w死亡。其余11只犬造模成功。微创切口组平均出血量为(15±6)mL,普通切口组平均出血量为(25±8)mL(P<0.05)。微创切口组手术平均时间为(20±6)min,普通切口组手术平均时间(34±3)min(P<0.05)。8w后普通切口组2只和微创组1只不需诱发即出现房颤。两组均可用burst刺激发出持续性房颤,微创切口组房颤持续时间(32±10)min,普通切口组房颤持续时间(37±11)min(P>0.05)。普通切口组及微创切口组起搏前后左、右心房超声面积均明显扩大:左房面积分别为(6.5±1.1)cm2和(12.3±1.6)cm2(P<0.01);右房面积分别为(4.6±1.3)cm2和(8.2±1.8)cm2(P<0.01)。结论微创切口具有手术时间短、出血量小、术中及术后并发症少等优点,是一种简便、高效、安全、稳定的慢性房颤动物模型制作方法。Objective To compare the differences of minimally invasive and ordinary incision in pacemaker implantation of canine chronic rapid atrial pacing in atrial fibrillation model to explore better canine modeling method to further investigate the mechanism of atrial fibrillation. Methods Twelve healthy adult dogs (1-2 years old) were randomly divided into the ordinary incision group (n =6) and minimally invasive incision group (n =6). Two groups were respectively used the ordinary incision and minimally invasive inci sion surgical implantation of a pacemaker electrode, so that established chronic atrial fibrillation model. We observed canine blood loss, operative time. High frequency pacing (400 bpm) with a permanent pace maker was given to two group dogs. The postoperative face electrocardiogram (ECG) and was recorded regular ge ly. neral condition of the dogs was observed by sur The occurrence of atrial fibrillation was recor ded and analyzed. ResultS' Twelve dogs were successful surgery. A dog of ordinary incision group was death because of pocket hematoma in the first week. 11 dogs were modeling successful. Minimally invasive recision group comp bleeding in minimall a Y red with the ordinary incision group, operative bleeding. The average amount of invasive incision group was (15 ± 6) mL, the ordinary incision group was (25 ±8) mL (P 〈0.05). The surgical time in minimally invasive incision group was (20±6) min, which was (34± 3) min ordinary incision group (P 〈0.05). The ordinary incision group after 8 weeks had the two occur atrial fibrillation without inducing, a minimally invasive group had only one occur atrial fibrillation without inducing. The ordinary incision and minimally invasive incision group could issued persistent atrial fibrilla tion by using burst stimulation.
