摘 要:目的探讨肌浆网钙ATP酶2a(SERCA2a)基因过表达对急性心房颤动(AF)兔心房有效不应期(AERP)的影响。方法 40只成年健康新西兰大白兔随机分4组:正常对照组(control组,n=8)、房颤组(AF组,n=8)、绿色荧光蛋白+房颤组(EGFP+AF组,n=12)、肌浆网ATP酶2a+绿色荧光蛋白+房颤组(SER-CA2a+EGFP+AF组,n=12)。control组仅植入电极不起搏;AF组、EGFP+AF组和EGFP+SERCA2a+AF组采用快速心房起搏(频率600次/min)制作急性AF模型,后两组起搏前30d分别向心包腔内注射携带EGFP和SERCA2a+EGFP基因的rAAV载体。转染30d,荧光显微镜下观察心肌组织绿色荧光表达情况,观察起搏0、4、8、12、24hAERP的变化。结果 EGFP+AF组兔心肌冰冻切片在荧光显微镜下可见弥漫绿色荧光,起搏4h后AF组和EGFP+AF组AERP较control组显著缩短(P<0.05),随起搏时间延长呈进行性加重,AERP在AF组和EGFP+AF组间无明显差异(P>0.05),SERCA2a+EGFP+AF组自起搏12h后AERP较AF组和EGFP+AF组缩短程度减轻(P<0.05)。结论以rAAV9为载体介导SERCA2a基因转导可有效逆转快速心房起搏兔心房肌的电重构,是一种潜在的治疗AF的方法。Objective To explore the effects of sarcoendoplasmic reticulum calcium ATPase2a (SERCA2a) gene on atrial effective refractory period (AERP) in atrial fibrillation (AF) rabbits by 24 h rapid atrial pa cing. Methods Forty New Zealand rabbits were randomly divided into four group: control group, AF group (n = 8), EGFP+AF group (n = 12) and SERCA2a+EGFP+AF group (n = 12). Control group was not paced, AF group, EGFP+ AF group and SERCA2a+EGFP+AF group underwent rapid atrial pacing with frequency of 600 beats per minute. Recombinant adeno-associated virus carrying enhanced green fluo rescent protein gene (rAAV9-EGFP) and recombinant adeno-associated virus carrying green fluorescent protein gene and SERCA2a gene (rAAV9-SERCA2a-EGFP) were respectively delivered into pericardium of the rabbits in the latter two groups by intrapericardial injection with a trans-diaphragmatic approach before rapid atrial pacing 30 d. Under the fluorescence microscope, expression of the green fluorescent protein was observed in the myocardial frozen section of rabbits in EGFP+AF group after thirty days gene transduction. The changes of atrial effective refractory period (AERP) were observed 0, 4,8, 12, 16 and 24 hours after pacing. Results Under the fluorescence microscope, widespread green fluorescence was observed in the myocardial frozen section of rabbits in EGFP+AF group. The AERP was markedly shortened (P %0.05) in AF group and EGFP+AF group %0.05). The AERP shortage showed aggravating than that in control group after pacing for 4 hours (P trend as the prolong of pacing time in AF group and EGFP+AF group compared with control group (P 〈0.05). There was no difference in the AERP be- tween AF group and EGFP+AF group (P)0.05). AERP was unmarkedly shortened in SERCA2a+EG- FP+AFgroup compared with AF group and EGFP+AF group after pacing for 12 hours (P 〈0.05).
