阿奇霉素联合头孢曲松钠治疗儿童难治性肺炎支原体肺炎疗效观察

摘　　要：目的评价阿奇霉素联合头孢曲松钠治疗儿童难治性肺炎支原体肺炎(RMPP)的疗效及安全性。方法 84例收治入院的RMPP儿童,以入院时间按1:3比例随机分为常规单药组(静脉滴注阿奇霉素,n=21)和联合用药组(静脉滴注阿奇霉素+头孢曲松钠,n=63)。以发热持续时间、咳嗽缓解时间和住院时间及肺部炎症总吸收率评估临床疗效,并观察不良反应。结果治疗后,联合用药组和常规单药组的总有效率分别为84.13%和57.14%,差异有统计学意义(P<0.05)。与常规单药组相比,联合用药组发热持续时间、咳嗽缓解时间及住院时间等明显较短(P<0.05);与常规单药组相比,联合用药组炎症总吸收率更高(73.02%对38.10%,P<0.05)。两组均无严重不良反应发生。结论阿奇霉素联合头孢曲松钠治疗儿童RMPP效果明显,不良反应少,安全可行。Objective To evaluate the efficacy and safty of azithromycin combined with ceftriaxone sodium in the treatment of refractory Mycoplasma pneumoniae pneumonia （RMPP） in children. Methods A total of 84 children hospitalized with RMPP were divided into two groups in the ratio of one to three according to admission time. The single therapy group （n=21） received intravenous administration of azithromycin, the combination therapy group （n=63） received intravenous administration of azithromycin and ceftriaxone sodium. The clinical efficacy with the pyretolysis time, cough relief time, hospitalization time and the absorption rate of inflammation were assessed, and tile drug adverse reactions were observed as well. Results After treatment, the overall response rate of combination therapy group and the single therapy group were 84.13% and 57.14% respectively, which was significant difference between two groups （P〈0.05）. The pyretolysis time, cough relief time and hospitalization time in combination therapy group were significantly shorter than those in single therapy group （P〈0.05）. The absorption rate of inflammation of combination therapy group was significantly higher than that in single therapy group （73.02% vs 38.10%, P〈0.05）. There were no serious drug adverse reactions in both two groups. Conclusion Azithromycin combined with ceftriaxone sodium in the treatment of RMPP can get better therapeutic efficacy with less adverse reaction.
