摘 要:目的探讨腹部袋状皮瓣术后外形及功能改善的解剖学基础。方法对临床上传统腹部袋状皮瓣及手部皮肤的解剖结构进行比较.针对传统手术方法的不足提出设计并改良。结果手部与腹部皮肤在解剖及生理上存在一定的差异。传统的腹部袋状皮瓣在手脱套伤的修复中有明显的缺点,为此我们进行了改进。采用腹部真皮下血管网袋状皮瓣修复全手脱套伤.更符合手部皮肤的解剖学结构及生理特点,使原有弃之可惜,留之无用的治疗手段成为目前保全肢体较好的手术方法。结论采用腹部真皮下血管网袋状皮瓣术治疗全手脱套伤可改善手部的功能及外形.且方法简单.对组织损伤也小。Objective To study an anatomical basis of the postoperative improved configuration and function of the abdominal pocket flap. Methods Compared anatomical structures of the traditional abdominal pocket flap with the hand skin, the distinctive defects of the traditional operative method was found. An improved method was thus designed. Results Certain anatomical and physiological differences in the skin structures were presented between the hand and abdomen. Several prominent defects in traditional abdominal poket flap to repair the degloved injury were also presented. A new improved abdominal subcutaneous poket flap with subdermal vascular netwrok method was thus designed to repair the hand degloved injury, this method was more conformed to the anatomical structures and physiological demends of the hand. Conclusion The pocket flap with subdermal vascular netwrok is made from the abdominal, that can improve postoperative hand configuration and function. It is simple and least traumatic.
