超声心动图对胎儿心律失常的诊断价值

摘　　要：目的探讨超声心动图在诊断胎儿心律失常方面的临床价值与效果。方法回顾本院120例确诊的胎儿心律失常患者的临床资料,并对其随访至产后1个月。结果 120例胎儿中,发生房性期前收缩84例,占70.00%;发生室性期前收缩15例,占12.50%;发生窦性心动过速10例,占8.33%;发生窦性心动过缓5例,占4.17%;发生室上性心动过速4例,占3.33%;发生心房扑动2例,占1.67%。120例心律失常胎儿,有4例因症状严重,家属选择终止妊娠,其余均自行好转或经治疗好转。结论超声心动图对于胎儿心律失常的诊断有可靠的价值,经检查大多数的胎儿心律失常表现为房性期前收缩和室性期前收缩,一般都是胎儿发育中的正常过程,经过一段时间的发育,一般都能恢复正常,不需要特殊的治疗措施。只有极少部分的胎儿心律失常需要采取治疗措施。Objective To probe into the clinical value and effect of ultrasonic cardiogram in the diagnosis of fetal cardiac arrhythmia.Methods Clinical data from 120 fetuses diagnosed as cardiac arrhythmia and admitted into our hospital were retrospectively analyzed as well as follow-up visit lasted till one month after delivery.Results Among 120 cases,the number of fetuses occurred premature atrial contraction,premature ventricular beats,nodal tachycardia,sinus bradycardia,supraventricular tachycardia,and atrial flutter were 84 cases（70.00%）,15 cases（12.50%）,10 cases（8.33%）,5 cases（4.17%）,4 cases（3.33%）,and 2 cases（1.67%）,respectively.Only 4 cases out of 120 cases due to critical conditions were terminated pregnancy based on their family＇s choices.The rest were in spontaneous improvement or got better after treat ment.Conclusion The diagnosis of fetal cardiac arrhythmia by ultrasonic cardiogram is reliable.By examination,the fre quently occurred symptoms of most fetal cardiac arrhythmia are premature atrial contraction and premature ventricular beats,which are the normal progress of fetal development.After a period of development,they will turn to be normal with out special treatment.Only few fetal cardiac arrhythmias need to take therapeutic measures.
