急性心肌梗死超早期及早期心律失常的特点分析

摘　　要：目的探讨急性心肌梗死(AMI)超早期及早期发生心律失常的特点。方法分析2007年3月～2009年3月本院收治的63例急性心肌梗死患者在超早期及早期发生的心律失常。结果 63例AMI患者出现心律失常52例,心律失常检出率为82.5%,其中超早期检出心律失常21例,早期检出心律失常31例。超早期中出现缓慢型心律失常15例,占超早期心律失常的71.4%;早期出现室性心律失常20例,占早期检出心律失常的64.5%。结论通过观察分析,急性心肌梗死患者的超早期(发病后6h内)及早期(发病后6～72h)心律失常发生率高,超早期以缓慢型心律失常为主,而早期则以室性心律失常为主。因此,应对急性心肌梗死患者加强监护和观察,及早发现心律失常,根据心律失常特点及早采取干预措施,尽量或避免心律失常,降低死亡率。Objective To investigate the characteristic of ultra-early and early arrhythmia in patients with acute myocardial infarction. Methods From March 2007 to March 2009 in our hospital, the clinical data of ultra-early and early arrhythmia in 63 patients with acute myocardial infarction were analyzed. Results Fifty-two cases were the arrhythmia in 63 patients with acute myocardial infarction, the arrhythmia detection rate was 82.5%, included 21 cases with ultra-early detection, 31 cases with early detection. 15 cases of slow-type arrhythmia in ultra-early stage, accounted for 71.4% of the ultra-early arrhythmia; 20 cases of ventricular arrhythmias in early stage, accounted for 64.5% of the early detection of arrhythmia. Conclusion The incidence of ultra-early （within 6 h after onset） and early （within 6-72 h after onset） arrhythmia in patients with acute myocardial infarction is high by observing and analyzing, ultra-early stage is mainly with slow-type arrhythmia, early stage is mainly with ventricular arrhythmias. Therefore, the response to the intensive care and observation of patients with acute myocardial infarction, early detection of arrhythmia, according to the arrhythmia characteristics early intervention measures, to minimize or avoid arrhythmias and reduce mortality.
