摘　　要：目的探讨ALT低水平慢性HBV感染者肝组织病理变化与HSC活化的相关性研究。方法①选取外周血中HBV-DNA定量≥103copies/mL、ALT≤3ULN的慢性HBV感染病例71例,行肝穿肝组织学检测;②采用HE染色检测肝组织的炎症活动度和肝纤维化程度;③用突触素(SYN)标记活化的肝星状细胞,SP法检测肝组织中阳性细胞的表达强度,在高倍镜下观察阳性细胞的表达,采用Image-ProPlus6.0彩色图像分析系统对图象进行分析。用积分光密度(IOD)值表示其在肝组织中表达的强度。结果①71例慢性乙肝病毒感染病例中,肝组织学完全正常(G0S0)者3例(4.23%),肝组织学存在不同程度损害者68例(95.77%),其中早期肝硬化3例(4.23%)。②不同损伤程度的肝组织中SYN阳性细胞的数量不同,所测得的IOD值有统计学差异(P<0.05)。③随着肝组织炎症活动加剧和肝纤维化程度加重,SYN表达就越显著。结论 HSC的活化与肝组织的炎症活动及纤维化成正相关。Objective To study the ALT low level chronic HBV infection liver tissue pathological changes and HSC activation correlation research.Methods ①Select the blood HBV-DNA quantitative or 103 copies/mL,ALT acuities 3 ULN chronic HBV infection cases 71 cases,hepatic wear liver histology detection;②HE dyeing detection by inflammation of the liver tissue activity and liver fibrosis degree;③Use synaptic hormone（SYN） mark activation of hepatic stellate cells,SP method to detect the liver tissue positive cell expression intensity,at high magnification to observe the expression of positive cells,the Image-Pro Plus 6.0 color Image analysis system for Image analysis.Using integral optical density（IOD） value said it’s in liver tissue in the strength of the expression.Results ①71 cases of chronic hepatitis b virus infection cases,liver histology completely normal（G0S0） is in 3 patients（4.23%）,liver histology there exists different degree impairer 68 cases（95.77%）,including early liver cirrhosis in 3 patients（4.23%）.②Different damage degree of liver tissue SYN in the number of positive cells are different,the measured value of the IOD was statistically difference（P0.05）.③Liver tissue inflammation and liver fibrosis increased activity degree aggravating,the significant SYN expression.Conclusion The activation of HSC and inflammation of the liver tissue activities and fibrosis into positive correlation.
