胺碘酮、普罗帕酮治疗老年肿瘤患者室上性心动过速疗效观察

摘　　要：目的探讨机械刺激无效后分别采取胺碘酮、普罗帕酮以及2种药物联合静脉注射终止老年肿瘤患者室上性心动过速(SVT)发作的疗效及安全性。方法将本院门诊和住院治疗的56例未能采取机械刺激兴奋迷走神经终止SVT的老年肿瘤患者,按就诊顺序随机分成胺碘酮组(32例)和普罗帕酮组(24例),分别缓慢静脉注射胺碘酮、普罗帕酮,若单一用药无法终止,则2种药物联合应用,比较终止SVT的有效性及不良反应。结果胺碘酮组治疗后有效25例,无效7例,有效率为78.1%;普罗帕酮组治疗后有效19例,无效5例,有效率为79.2%,两组治疗SVT的疗效比较,差异无统计学意义(P>0.05)。单一用药无法终止共12例,联合用药后有效11例,无效1例,总有效率达98.2%。未观察到治疗后有严重的不良反应。结论 2种药物都能有效地终止SVT,若单一用药无效后,联合用药仍安全且更有效。Objective To evaluate the effectiveness and security of Amiodarone,Propafenone and Amiodarone merged Propafenone by intravenous injection to terminate the room tachycardia（SVT） attack for elderly patients with tumor,after invalid mechanical stimulation.Methods Fifty-six older patients hospitalized in out-patients in our hospital who failed to take mechanical stimulation excited vagus nerve to terminate SVT were randomly divided into two groups according to the order of visiting：amiodarone group（32 subjects）,Propofenone group（24 subjects）.They were given Amiodarone,Propafenone respectively by intravenous injection.And if single drug was invalid,they were given Amiodarone merged Propafenone.The effectiveness of terminaing SVT and adverse reaction were compared.Results After the treatment,Amiodarone group showed that the effective were 25 patients,ineffective were 7 patients,and the effective rate was 78.1%.Propafenone group showed that the effective were 19 patients and ineffective were 5 patients,and the effective rate was 79.2%.There was no significant difference between two groups（P 〈0.05）.That were 12 cases of single drug which could＇t terminate.The combination drug group showed that the effective were 11 patients and ineffective were 1 patient,the total effective rate was 98.2%.No ill-effects had been detected during the process.Conclusion These two drugs of Amiodarone and Propafenone are both effective to terminate SVT,if a single drug to be null and void,the combined treatment is still safe and more effective for treatment.
