摘 要:目的:观察糖尿病患者24 h尿微量白蛋白(24 h-mAlb)与随机尿白蛋白/肌酐比值(ACR)和晨尿ACR的相关性,探讨采用随机尿ACR代替24 h-mAlb用于糖尿病早期肾损伤筛查的可行性。方法:收集120位住院患者24 h尿、晨尿、随机尿的尿液标本,用透射免疫比浊法测定24 h-mAlb,用散射免疫比浊法测定ACR。结果:晨尿组ACR与随机尿组ACR无统计学差异(P>0.05),晨尿组ACR与24 h-mAlb呈正相关(r=0.714 5,P<0.01),相关性良好;随机尿组ACR与24 h-mAlb呈正相关(r=0.823 5,P<0.01),相关性良好。结论:测定随机尿ACR方法简单易行,尤其是方便门诊病人,可代替24 h-mAlb用于糖尿病早期肾损伤筛查。Objective: To examine the feasibility of using random urine ACR instead of 24 h urinary albumin(24 h-mAlb) for early renal injury screening by observing the correlation between the 24 h-mAlb and random urine ACR and the morning urine ACR of diabetic pa- tients. Methods: The 24 h urine, morning urine, random urine fluid samples from 120 hospitalized patients were collected, then the 24 h-mAlb was measured by transmission immune turbidimetric method, and the ACR was measured by scattering immune turbidimetric method. Results: There was no significant difference in ACR compared morning urine group with random urine group (P〉0.05); It showed a positive correlation between the morning urine group and 24h-mAlb (r=0.714 5, P〈0.01); The ACR of random urine group was positive- ly correlated with 24 h-mAlb (r=0.823 5, P〈0.01). Conclusion: The random urine ACR could instead of 24 h-mAlb for early renal injury screening, which is worth developing for it is simple, convenient for patients, especially outpatients.
