95例急诊机械通气患者抗菌药物应用分析

摘  要:目的分析我院急诊抢救室中机械通气患者抗菌药物的应用情况。方法对我院95例急诊机械通气患者抗菌药物的应用情况进行回顾性调查。结果我院急诊进行机械通气的危重患者中,抗菌药物的使用率达76.8%,其中联合应用抗菌药物占使用病例数的46.6%。抗菌药物使用率高的病种依次为颅脑外伤(100.0%)、慢性阻塞性肺病(100.0%)、各种中毒性疾病(81.8%)、心肺复苏术后(80.0%)。联合应用抗菌药物比例较高的病种分别是慢性阻塞性肺病(83.3%)和心肺复苏术后(75.0%)。使用抗菌药物的种类以使用头孢菌素最多,占使用病例数的71.2%;β内酰胺酶抑制剂、氟喹诺酮类也较多使用,分别占31.5%和24.7%。结论急诊机械通气患者应合理使用抗菌药物,根据患者的病情确定是否给予抗菌药物并注意个体化给药。Objective To investigate the application status of antibacterials in the patients with mechanical ventilation in the resuscitation room of emergency department of our hospital. Methods The use of antibacterials in 95 emergency pa- tients with mechanical ventilation was analyzed retrospectively. Results Of the total 95 cases, 76.8% received antibacterials. 46.6% of the case of received antibacterials used antibiotics in combination. The diseases of the higher application of antibacterials were craniocerebral trauma (100.0%), chronic obstructive pulmonary disease (100.0%), toxic disease (81.8%), post cardiorespiratory resuscitation (80.0%). The diseases of the higher proportion of used antibiotics in combination were chronic obstructive pulmonary disease (83.3%), post cardiorespiratory resuscitation (75.0%). Cephalosporins showed the highest proportion (71.2%) of the case of received antibacterials. The proportion of β-1actamase inhibitors and Fluoro- quinolones were 31.5%, 24.7% in turns. Conclusion The emergency patients with mechanical ventilation should use an- tibacterials rationally. Individualized administration according to the pathogenetic condition should be emphasized.
