摘　　要：目的探讨脑血吸虫病MRI表现特征及诊断价值。方法回顾性分析8例经手术病理或临床证实的脑血吸虫病的MRI表现。结果 3例单发;5例多发分布于额顶颞叶皮层和皮层下区,其中2例同时发生于小脑,1例病灶同时发生于基底节区。T1WI虫卵肉芽肿病灶为斑点状或结节状等、略低信号,同周围低信号水肿分界欠清,T2WI为等稍高信号,与水肿一起形成佛手或手握球征,1例伴发血肿,1例伴发梗塞;增强扫描呈斑片、结节样强化,病灶多中心分布、簇状堆积,延时扫描部分结节灶融合,其中4例病变区见到条状强化,2例见到环形强化,4例病灶邻近脑膜示条状、脑回样强化。灶周水肿重而占位效应相对轻。治疗后复查病灶缩小、消失,水肿消退。结论 MRI能清晰显示脑血吸虫病的特征。MRI增强扫描对脑血吸虫早期诊断至关重要,同时MRI检查也是随访及观察疗效的重要方法。Objective To study MRi characteristics and diagnostic value of cerebral schistosomiasis. Methods MRI findings of 8 cases with cerebral schistosomiasis in the pathologically or clinically proved were analyzed retrospectively. Results Single masses were found in 3 cases,and multiple masses in 5 cases,distributed in the temporal lobe cortex forehead and st, bcortical region,of which 2 cases occur at the same time in the cerebelhuu, and 1 case occur at the same time in the basal ganglia. TIWI worm eggs granulomatous lesion for the speckled or nodular equal and slightly lower intense of the MRI, with its low signal edema boundary owe clear, T2WI t：or a little high and equal intense, edema together form lor ＂gigantic hand＂ or ＂hand ball sign, 1 case with helnatoma, 1 cases with illfarction＂ Enhance scanning appear a spot piece and nodules shape strengthening, focal appear more central distribution and clumps accmnulation, delay scanning part focal nodules fusion, the linear enhancement was seen in disease area of 4 cases, ring-like enhancement was seen in disease area of 2 cases, focals near meninx of 4 cases appear linear and gyms enhancement. perilesional edema heavy but local mass effect relatively clear. After treatment focals were narrow and disappear,edema is gone. Conclusion The characte,istics of cerebral schistoso- miasis can be showed clearly to MR1.MRI enhance brain scan to the early diagnosis is crucial, and MRI check is also observe the curative effect of follow-tp and important way.
