穿腹壁缝扎法经脐单孔腹腔镜精索内静脉曲张高位结扎术临床报告(附50例)

摘　　要：目的探讨穿腹壁缝扎法在经脐单孔腹腔镜精索内静脉曲张高位结扎术中的临床意义。方法总结2010年1月至2011年6月采用穿腹壁缝扎法经脐单孔腹腔镜精索内静脉曲张高位结扎术治疗的50例62侧病例资料。结果手术时间平均25(20～35)min。50例62侧精索内静脉曲张患者全部治愈出院,均无近远期并发症发生,随访5～18月,无阴囊肿胀、复发,无睾丸萎缩等并发症。术后复查B超、精液检查各项指标较术前均有不同程度改善。结论穿腹壁缝扎法经脐单孔腹腔镜精索内静脉曲张高位结扎术创伤小,手术时间短,效果显著,恢复快,无明显并发症,是治疗精索内静脉曲张一种较为理想的手术方式,值得临床推广应用。Objective To summarize the surgical experiences of transumbilical single-port laparoscopic spermatic vein ligation for the treatment of primary varicocele. Methods The clinical data were analyzed ret- rospectively in 50 patients（62 sides） underwent transumbilical single-port laparoscopic spermatic vein ligation from January 2010 to June 2011. Results The operation time was 20 to 35 minutes, with 25 minutes in aver- age. All cases were followed up for 5 ～ 18 months, and no scrotum swelling, recurrence or testicle atrophy was observed. The ultrasonography and the seminal fluid inspects showed varying degree improvement. Conclu- sions It demonstrates the advantages of mini-tranma, short operation time, remarkable effect, quick recovery and no significant complication of the transumbilical single-port laparoscopic spermatic vein ligation, which is recommended as an ideal operation for the treatment of varicose veins, and is worthy to be popularized.
