老年良性前列腺增生症与高敏C反应蛋白相关性研究

摘　　要：目的：探讨血清高敏c反应蛋白（hs-CRP）水平与老年良性前列腺增生症（benign prostatic hyperplasia，BPH）间的关系。方法：选取我院老年病科门诊及住院的96例BPH患者，询问病史，测定身高、体质量，计算体质量指数（BMI），并检测其血清hs-CRP和前列腺特异性抗原（PSA）水平，经腹壁B超测定前列腺体积（PV）。采用国际前列腺症状评分（IPSS）评估下尿路症状（LUTS）。结果：根据PV，分为〈30mL组（36例）、30～50mL组（32例）、〉50mL组（28例），结果显示3组hs．CRP水平分别为（1．64±0．34）mg／L、（1．81±0．25）mg／L和（2．59±0．29）mg／L。随着PV增大．hs-CRP水平呈增高趋势，〉50mL组的hs—CRP水平较〈30mL组、30～50mL组明显升高（P〈0．05）。根据IPSS评分．分为〈7分组（39例）、8～19分组（42例）及〉20分组（15例），结果显示3组hs—CRP水平分别为（1．61±0．33）mg／L、（1．98±0．25）mg／L和（2．76±0．43）mg／L，8—19分组、〉20分组明显高于〈7分组（P分别〈0．05、〈0．01），〉20分组明显高于8～19分组（P〈0．01）。IPSS与hs－CttP呈正相关（r=O．436，P〈O．01），但前列腺体积与Hs—CRP间无相关性（P〉0．05），hs－CRP与血清PSA水平亦无关（P＞0．05）。结论：血清hs－CRP水平与老年BPH的进展有关．可能是预测和影响LUTS的因素之一。Objective To study the relationship between high sensitive C- reactive protein （hs-CRP） and benign prostatic hyperplasia （BPH） in elderly men. Methods BPH outpatient or inpatient in Department of Geriatrics of Ruijin hospital from August to October 2011 were enrolled. Medical history, biochemical assays including hs-CRP and prostate specific antigen （PSA）, prostate volume （PV） measured by ultrasound, and lower urinary tract symptoms （LUTS） evaluated by International Prostate Symptom Score （IPSS） were analyzed. Blood pressure, body weight and height were measured, and body mass index （BMI） was calculated. Results ① Patients were divided into three groups according to PV： PV〈30 mL, PV 30-50 mL , PV〉50 mL groups. Serum hs-CRP level was higher in PV〉50mL group （P〈0.01）. ② According to IPSS, the patients were divided into three groups： IPSS〈7 scores, IPSS 8-19 scores, IPSS〉20 scores. Serum Hs-CRP was significantly higher in IPSS 8-19 scores and IPSS 〉 20 scores groups than in IPSS 〈7 scores group （P〈0.05,〈0.01 respectively）. ③Serum hs-CRP was positively correlated with IPSS （r=0.435,P〈0.01）. Conclusions Serum hs-CRP is related to the development of BPH in eider men, and hs-CRP may be a good marker to predict and influencing the develot）ment of LUTS.
